چکیده

مقدمه: آناستوموزهای تراشه از اعمال جراحی شایع روی Upper respiratory airway هستند که به دلیل ایجاد تنگی در محل آناستوموز، به دنبال راه‌های جایگزین آن هستند. این آناستوموزها، در شیوه‌ی معمول، با استفاده از بخیه‌هایی با نخ ویکریل یا استاپلر انجام می‌شوند. مطالعه‌ی فعلی به بررسی امکان استفاده از چسب فیبرینی جدید در آناستوموز تراشه جهت کاهش زمان عمل و عوارض آن پرداخت.روش‌ها: این مطالعه به روش Pure experimental trial بر روی 8 قلاده سگ که از نظر نژاد و وزن و جنس مشابه بودند، انجام شد. در دو نوبت، توسط یک برش گردنی 2 رینگ از تراشه‌ی گردنی سگ‌های مورد مطالعه برداشته شد. آناستوموز در نوبت اول توسط چسب فیبرینی جدید بعد از 3 بخیه‌ی نگهدارنده انجام شد (گروه مورد). بعد از دو هفته در مرحله‌ی دوم، پس از برداشتن (Resect) ناحیه‌ی مذکور، آناستوموز مجدد به صورت کلاسیک و با 12 بخیه انجام شد (گروه شاهد). پس از 2 هفته، نمونه‌ها تحت جراحی مجدد قرار گرفتند و محل‌های آناستوموز جهت بررسی پاتولوژی برداشته (Resect) شد و نمونه‌ها تحت آناستوموز با شیوه‌ی کلاسیک قرار گرفتند. داده‌ها با نرم‌افزار SPSS نسخه‌ی 17 مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: میانگین زمان انجام آناستوموز در گروه مورد 94/5 و در گروه شاهد 75/9 دقیقه بود (05/0 > P). همچنین میانگین قطر تراشه در آناستوموز با فیبرین به طور معنی‌داری بیش از گروه شاهد بود (میانگین گروه مورد 59/25 میلی‌متر و میانگین گروه شاهد 14/24 میلی‌متر). در بررسی نتایج از نظر ماکروسکوپی اختلافی بین دو شیوه‌ی آناستوموز مشاهده نشد و چسبندگی به طور کامل رخ داده بود. مرگ و میر و نشت (Leak) و میکروآبسه مشاهده نشد. در بررسی پاتولوژی اتصال در تمام نمونه‌‌ها به صورت کامل رخ داده بود.نتیجه‌گیری: با توجه به کوتاه‌تر بودن زمان آناستوموز به شیوه‌ی چسب فیبرینی جدید و نیز عدم اختلاف نتایج پاتولوژی و عوارض بین دو شیوه‌، چسب فیبرینی جدید جهت انجام آناستوموز تراشه با اطمینان بالا می‌تواند مورد استفاده قرار گیرد.

Background: Tracheal anastomoses are common parts of upper respiratory airway surgeries. Anastomosis is currently performed using Vicryl sutures or staples. Due to the high prevalence of postoperative tracheal stenosis, efforts have been made to find alternative methods. This study compared the use of new fibrin glue in tracheal anastomosis with the classic method.Methods: Eight dogs of similar sex, weight, and race were included in this experimental trial. The surgery was performed in two different stages. In each stage, two rings of trachea were resected. In the first stage, anastomosis was performed with the new fibrin glue. In the second stage, the anastomosis was performed classically. After two weeks, the regions of anastomosis were resected for pathologic evaluations.Findings: There was a statistically significant difference in the mean duration of operation between the two methods (5.94 minutes using the fibrin glue method and 9.75 minutes using the classic method) and the mean tracheal diameter after anastomosis (25.59 mm and using the fibrin glue method and 24.14 mm using the classic method). In gross evaluation, there was not a significant difference between two different types of anastomosis. According to microscopic study, attachment was performed completely in all cases.Conclusion: Anastomosis with new fibrin glue was faster and with less postoperative stenosis and fewer complications compared to the classic method. Therefore, we claim this method as a suitable substitute for the classic method of tracheal anastomosis.
