بررسی نقش رینیت آلرژیک در ابتلا به اوتیت سروز

چکیده

مقدمه: اوتیت سروز شایع‌ترین علت قابل پیش‌گیری از دست دادن شنوایی در بچه‌ها در سنین قبل از مدرسه ‏‌در کشورهای توسعه یافته می‌باشد که هزینه‌های گزافی بر سیستم بهداشتی تحمیل می‌کند. شناخت عوامل مؤثر در افزایش بروز و شیوع این بیماری جهت کنترل مؤثرتر آن کمک کننده است. در این مطالعه ارتباط بین اوتیت سروز و رینیت آلرژیک، به عنوان یک عامل مستعد کننده، بررسی شد.روش‌ها: 52 بیمار مبتلا به اوتیت سروز تأیید شده در شرح حال و معاینه‌ی بالینی با اتوسکوپ تیمپانومتری با 52 فرد سالم و غیر مبتلا به اوتیت سروز بر اساس معاینه‌ی بالینی و تمپانومتری از نظر وجود ائوزینوفیل در ترشحات بینی مورد مطالعه و مقایسه قرار گرفتند. از هر دو گروه، توسط سواپ، گستره‌ای از ترشحات بینی بر روی لام ایجاد و جهت بررسی ائوزینوفیل رنگ آمیزی شد. در صورتی که 20 درصد از پلی‌مورفونوکلوئرهای موجود را ائوزینوفیل تشکیل می‌داد، نتیجه مثبت تلقی شد. یافته‌ها: 6 مورد (‌5/11 درصد) از گروه بیماران، ‌رینیت آلرژیک هم‌زمان داشتند و تنها یک مورد (9/1 درصد‌) از گروه شاهد مبتلا به رینیت آلرژیک بودند که این اختلاف بر اساس آزمون 2χ (05/0 > P) و آزمون دقیق فیشر (028/0 = ‌P)‌ معنی‌دار بود.نتیجه‌گیری: نتایج مطالعه مطرح کننده‌ی نقش رینیت آلرژیک در ایجاد برخی موارد اوتیت سروز بود.

چکیده [English]

Background: Otitis Media with effusion is a common cause of conductive hearing loss in children in school age and its pathogenesis is still under debate. In some studies a roll for allergic rhinitis has been suggested.Methods: In this study we investigated two groups (52 patients with otitis media with effusin and 52 healthy persons) for allergic rhinitis based on history, physical examination and nasal discharge smear.In this study, If 20% of PMN cells of nasal smear were eosinophil, that sample was positive for allergic rhinitis.Finding: Allergic was found in 6 persons of study group and only one person of control group. This difference was significant (P < 0.05 in chi square test and P = 0.028 in fisher test).Conclusion: Our study suggests that allergic rhinitis may play a role in a subset of patients with otitis media with effusin.
