بررسی مقایسه‌ای عوارض زخم در برش‌ جراحی به روش‌های خط وسط (Midline) و L معکوس در اعمال جراحی لاپاراتومی

چکیده

مقدمه: انتخاب برش جراحی برای لاپاراتومی به عوامل متعددی نظیر ناحیه‌ی نیازمند آشکار شدن (Exposure)، اورژانسی بودن عمل و تشخیص جراح بستگی دارد. این مطالعه، با هدف مقایسه‌ی عوارض شایع زخم جراحی در دو روش برش جراحی خط وسط (Midline) و L معکوس انجام شد تا با شناخت نقاط قوت و ضعف هر یک از روش‌ها، بتوان در شرایط مختلف به تناسب نیاز از آن‌ها استفاده نمود.روش‌ها: این مطالعه به صورت مقطعی بر روی 68 بیمار که طی سال‌های 95-1394 در بیمارستان الزهرای (س) اصفهان تحت عمل لاپاراتومی قرار گرفته بودند، انجام شد. 34 نفر از این بیماران، با روش برش جراحی Midline و 34 نفر با روش برش جراحی L معکوس تحت عمل قرار گرفته بودند. بیماران در طی مدت مطالعه از نظر شدت درد بعد از عمل، عفونت زخم، فتق برشی و بیرون ریختن احشای شکم در اثر باز شدن فاشیا، بررسی و مقایسه گردید.یافته‌ها: بین دو گروه تفاوت معنی‌داری از نظر فتق برشی، عفونت زخم و بیرون ریختن احشای شکم در اثر باز شدن فاشیا مشاهده نشد (050/0 < P برای همه‌ی موارد). نمره‌ی شدت درد نیز در دو گروه مشابه بود.نتیجه‌گیری: با توجه به مشابه بودن عوارض زخم در دو گروه، می‌توان در شرایط مختلف و بسته به نیاز از دو روش مورد مطالعه استفاده کرد.

Background: The selection of incisions for laparotomy depends on a number of factors such as the area that needs to explore, being emergency or elective (non-emergency), and ultimately, surgeon preference. This study aimed to compare the complications of surgical wound in two methods of midline and reverse L. incision in order to recognize the complications and benefits in any circumstances.Methods: This cross-sectional study was performed on 68 patients who underwent laparotomy in Alzahra hospital in Isfahan City, Iran, during the years 2015 and 2016. 34 patients were enrolled in midline incision group, and 34 patients underwent reverse L incision. During the study, the severity of postoperative pain, wound infection, incisional hernia, and dehiscence were compared.Findings: There was no significant difference between the two groups in terms of incisional hernia, wound infection, and dehiscence (P < 0.05 for all). Pain severity was similar in both groups, too.Conclusion: Due to the similarity of wound complications in the two groups, two different methods can be used in different conditions depending on need.
