بررسی تأثیر مکمل‌یاری پودر گیاه شوید بر مقاومت انسولینی و نشانگرهای التهابی در بیماران مبتلا به دیابت نوع 2

چکیده

مقدمه: بیماری دیابت اختلالی است که با بروز بسیاری از بیماری‌های مزمن ارتباط دارد. افزایش سطوح سیتوکین‌های التهابی، از جمله اینترلوکین 6 (6-IL یا 6-Interleukin)، عامل نکروز توموری-آلفا (TNF-α یا Tumor necrosis factor alpha) و پروتئین واکنشی C (hs-CRP یا High-sensitivity C-reactive protein)، بر حساسیت انسولینی تأثیر می‌گذارد. در سال‌های اخیر، استفاده از گیاهان سنتی به دلیل عوارض جانبی پایین، رواج بیشتری یافته است. مطالعه‌ی حاضر با هدف تعیین تأثیر مکمل‌یاری پودر گیاه شوید بر مقاومت انسولینی و نشانگرهای التهابی در بیماران مبتلا به دیابت نوع 2 انجام شد.روش‌ها: این مطالعه‌‌ی کارآزمایی بالینی دو سو کور، بر روی 60 بیمار مبتلا به دیابت نوع 2 در شهر تبریز انجام شد. افراد حایز شرایط، به صورت تصادفی به دو گروه مورد و شاهد تقسیم شدند. افراد گروه مورد، روزانه 3/3 گرم پودر گیاه شوید و افراد گروه شاهد، مقادیر مشابه پودر نشاسته را به مدت 8 هفته دریافت کردند. در ابتدا و انتهای مطالعه، غلظت انسولین ناشتا، 6-IL و TNF-α با استفاده از روش ELISA (Enzyme-linked immunosorbent assay) و hs-CRP با استفاده از اسپکترومتری اندازه‌گیری شد. شاخص مقاومت انسولینی با استفاده از فرمول گلوگز ناشتا (mg/dl) × انسولین ناشتا (μU/ml) /405 به دست آمد.یافته‌ها: کاهش معنی‌داری در غلظت انسولین ناشتا و سیتوکین‌های التهابی 6-IL، TNF-α و hs-CRP در گروه مورد مشاهده شد (050/0 > P). پس از تعدیل بر روی مقادیر پایه و متغیرهای مخدوشگر، این معنی‌داری تأیید شد. شاخص مقاومت انسولینی در هر دو گروه تغییرات آماری معنی‌داری نشان نداد.نتیجه‌گیری: پودر گیاه شوید، تأثیر مفیدی در کاهش نشانگرهای التهابی و غلظت انسولین ناشتا داشت. با این وجود، انجام مطالعات بیشتر برای تأیید نتایج به دست آمده پیشنهاد می‌گردد.

چکیده [English]

Background: Diabetes mellitus (DM) is associated with many chronic disorders. Increasing the concentration of circulatory cytokines such as interleukin 6 (IL-6), tumor necrosis factor alpha (TNF-α) and high-sensitivity C-reactive protein (hs-CRP) can effect on the insulin sensitivity. Recently, the use of herbs as complementary and alternative medicine, due to low level of side effects, has been prevailed. This study was designed to investigate the effects of Anethum graveolens supplementation on the insulin resistance and inflammatory biomarkers in patients with type 2 diabetes.Methods: This randomized-double blind-placebo controlled study was conducted on 60 patients with type 2 diabetes in Tabriz, Iran. The eligible patients randomly divided to two intervention and control groups. Intervention group received 3.3 g/day powder of Anethum for 8 weeks and control group received the same amounts of starch. At the onset and end of the study, serum concentrations of insulin, IL-6 and TNF-α was determined using the enzyme-linked immunosorbent assay (ELISA) method; and hs-CRP was measured using spectrometry. The homeostatic model assessment of insulin resistance (HOMA-IR), as an index of insulin resistance, was calculated as fasting concentrations of blood glucose (mg/dl) × fasting insulin (μU/ml)/405.Findings: Serum concentration of insulin, IL-6, TNF-α and hs-CRP decreased significantly at the end of study in intervention group (P < 0.05). This decreasing was confirmed after adjusting for baseline and confounding factors. Changes in HOMA-IR were not significant in none of the groups.Conclusion: Supplementation of Anethum graveolens had beneficial effects on the concentration of insulin concentration and inflammatory biomarkers. However, further clinical studies are suggested to confirm the results.
