بررسی عملکرد حرفه‎ای داروسازان در برخورد با نسخ دارویی در داروخانه‌های شهر اصفهان

چکیده

مقدمه: اشتباهات احتمالی نسخ در تجویز دارو، از قبیل تداخلات دارویی و یا تجویز داروی نامناسب، اگرچه ممکن است اجتناب ناپذیر باشد، لیکن گاهی می‌تواند عواقب حادی به همراه داشته باشد. در این مطالعه، میزان انجام عملکرد حرفه‌ای داروسازان شهر اصفهان، در خصوص تشخیص و اصلاح اشتباهات نسخه مورد بررسی قرار گرفت.روش‌ها: یک بیمار فرضی آموزش دیده، به داروخانه مراجعه و نسخه‌ای که حاوی تداخلات دارویی، دوز غیر متعارف دارو و داروهای دارای عوارض جانبی بود را به داروساز مسؤول فنی تحویل داده، با پرسیدن سؤالات مربوط به نسخه، نحوه‌ی برخورد داروساز با این اشتباهات را مورد ارزیابی قرار می‌داد. پژوهشگر به دنبال آن بود که بداند داروساز در خصوص آثار جانبی دارو، میزان نامناسب بودن دارو برای بیمار فرضی، تداخلات دارویی موجود در نسخه و نحوه‌ی مصرف دارو چقدر مهارت داشته، به آن‌ها توجه می‌نماید.یافته‌ها: از 146 داروخانه‌ی مورد بررسی، در 136 مورد (1/93 درصد) داروساز حضور داشت و از این تعداد، در 2/82 درصد موارد (120) حضور فعال وی مشهود بود. 99 نفر (8/67 درصد) از نامناسب بودن دارو برای بیمار اطمینان حاصل کردند که 96 نفر (8/65 درصد) آن‌ها، به روش‌های مختلف مانند عودت نسخه و یا عدم تحویل دارو به بیمار، وی را به مشورت با پزشک تشویق کرده، از او خواستند تا دوباره به پزشک خود مراجعه نماید و پس از اطمینان از صحت نسخه، جهت تحویل آن به داروخانه باز گردد. 4 تداخل دارویی مهم در نسخه گنجانده شده بود که 121 نفر از داروسازان هیچ‌گونه واکنشی نسبت به این تداخلات از خود نشان ندادند (9/82 درصد)؛ 3 نفر (1/2 درصد) یک تداخل و 2 نفر نیز (4/1 درصد) دو تداخل دارویی را تذکر دادند.نتیجه‌گیری: : اگرچه داروسازان از نحوه‌ی مصرف داروها به خوبی مطلع هستند و به آن در هنگام تحویل دارو توجه می‌نمایند، لیکن تداخلات دارویی و آثار جانبی داروها از مواردی است که به آن کمتر توجه می‌شود. پیشنهاد می‌گردد، در برنامه‌ی آموزشی دانشکده‌های داروسازی و همچنین، در برنامه‌های بازآموزی برای دانش‌آموختگان، این موارد بیشتر مورد توجه واقع گردد.

Background: Pharmacists, in community pharmacy, play an important role in public health as they serve as guardians of the accuracy of dispensed prescription. Although the assessment of professional competence for this profession is a difficult task, however, evaluation of this profession may help for giving better services and is needed for safe use of medications. The aim of this study was evaluation of dispensing accuracy and counseling provided by community pharmacists.Methods: During the summer of 2009, a total of 146 community pharmacists in Isfahan city, Iran, were randomly selected for the study. Community pharmacies were selected as the majority of prescriptions dispensed by this kind of pharmacies. Two trained shoppers presented a new prescription order to each randomly selected pharmacy to collect the objective data. The shoppers were instructed on how to make questions from the pharmacist to evaluate the pharmacist knowledge about the dosage, interactions or side effects of prescribed medicines.Findings: In this study of dispensing accuracy and counseling, 136 pharmacists (93%) were present and 120 of them (82%) were actively participated in dispensing and counseling. About 67.8% (99 pharmacists) of pharmacists were caught the inaccuracy of prescribed drug for the patient; 65.8% (96 pharmacists) did not deliver the drug to the patients and encouraged them to consult with their doctor. Four significant drug interactions were included in the prescription; 121 pharmacists (82% of) did not shown any responses to them.Conclusion: Although pharmacists are well-informed about the drugs given when delivering drugs, but drug interactions and side effects of drugs are cases where pharmacists should consider more.
