بررسی نتایج درمانی تزریق متیل پردنیزولون استات به همراه گچ‌گیری در مقایسه با تزریق متیل پردنیزولون استات به تنهایی در بیماران مبتلا به دکورون

چکیده

مقدمه: اجماع نظر واحدی درباره‌ی درمان مناسب در درمان بیماری دکورون وجود ندارد. این مطالعه در جهت مقایسه‌ی تزریق متیل پردنیزولون استات به همراه گچ‌گیری در برابر تزریق متیل پردنیزولون استات به تنهایی در درمان بیماری دکورون طراحی و اجرا شد.روش‌ها: در این کارآزمایی بالینی تصادفی انجام شده، 88 بیمار مبتلا به دکورون از بین بیمارانی که از شهریورماه 1392 تا شهریورماه 1393 به بیمارستان الزهرا (س) و مطب خصوصی پژوهشگران در شهر اصفهان مراجعه کرده بودند، انتخاب شدند. این بیماران به صورت تصادفی به دو گروه تقسیم شدند. در گروه اول تزریق mg 1 متیل پردنیزولون استات در اولین کمپارتمان اکستانسور مچ دست به همراه گچ‌گیری به مدت سه هفته انجام شد و در گروه دوم تنها تزریق mg 1 متیل پردنیزولون استات صورت گرفت. بیماران از نظر شدت درد و فراوانی نسبی تندرنس و آزمون Finkelstein ارزیابی شدند.یافته‌ها: شدت درد و فراوانی نسبی تندرنس و آزمون Finkelstein در هر دو گروه مشابه هم و به طور قابل توجهی کم شده بود.نتیجه‌گیری: به نظر می‌آید هر دو روش درمانی به صورت مساوی مؤثر هستند؛ اما به علت این که گچ‌گیری برای بیماران ناراحتی ایجاد می‌کند، از این رو تزریق کورتون به تنهایی در درمان بیماری دکورون بهتر از استفاده از تزریق کورتون به همراه گچ‌گیری است.

چکیده [English]

Background: There is no consensus in the treatment of de Quervain's disease. This study was performed to compare the methylprednisolone acetate injection with thumb spica cast versus methylprednisolone acetate injection alone for treatment of de Quervain tendinitis.Methods: In this randomized clinical trial, 88 patients with de Quervain's disease were selected from patients who referred to Al-zahra Hospital and the researchers’ private offices in Isfahan, Iran. The patients were randomly assigned into 2 groups. 1 mg methylprednisolone acetate injection in first dorsal compartment of the wrist accompanied with thumb spica cast for 3 weeks was the therapeutic method utilized in the first group; methylprednisolone acetate injection alone was performed in the second group. The patients were evaluated in terms of pain intensity and the relative prevalence of tenderness and Finkelstein's test.Findings: Pain intensity and prevalence of tenderness and Finkelstein's test in both groups were similar and become lower significantly.Conclusion: It seemed that both therapeutic methods are effective equally; but as casting makes trouble for the patients, so corticosteroid injection alone is better than corticosteroid with casting in treatment of de Quervain's disease.
