ارتباط میان سطح سرمی منیزیم و پروفایل چربی در بیماران تحت دیالیز صفاقی و همودیالیز

چکیده

مقدمه: برخی مطالعات نشان داده است که منیزیم بر ایجاد وگسترش فرایند آترواسکلروزیس با ایجاد تغییردر فرایندهای التهابی آسیب ناشی از اکسیداسیون سلولى، افزایش سطح LDL (Low-density lipoprotein) سرمی و تحریک عوامل رشد دخالت می‌کند. منیزیم به طور مستقیم در بالا بردن سنتز لیپوپروتئین‌ها اثر ندارد، اما ممکن است در تنظیم آنزیم‌های مسؤول سنتز لیپوپروتئین‌ها در سطح کبدی دخالت داشته باشد که بیشتر باعث تغییر متابولیسم تری گلیسیریدها می‌گردد. این مطالعه با هدف بررسی سطح سرمی منیزیم و پروفایل چربی در بیماران تحت دیالیز صفاقی و همودیالیز انجام شد.روش‌ها: در این مطالعه‌ی توصیفی- تحلیلی، 110 بیمار تحت دیالیز صفاقی و همودیالیز در سال 1392 در زاهدان بررسی شدند. نمونه‌گیری به صورت آسان و در دسترس بود و بیماران در دو گروه تحت دیالیز صفاقی و تحت همودیالیز قرار گرفتند. ارتباط منیزیم و سطح لیپیدهای خون بیماران بررسی شد.یافته‌ها: 51 نفر از بیماران (4/46 درصد) مرد و 89 نفر (6/53 درصد) زن بودند و میانگین سنی آنان 7/14 ± 1/46 سال بود. در بیماران تحت دیالیز صفاقی، ضریب همبستگی بین منیزیم و کلسترول فقط در گروه مصرف کننده‌ی داروهای آنتی لیپید معنی‌دار بود (003/0 = P). میزان سطح منیزیم با LDL-C (Low-density lipoprotein cholesterol) در بیماران تحت دیالیز صفاقی فقط در گروه مصرف کننده‌ی داروهای آنتی لیپید ارتباط داشت (036/0 = P). در بیماران تحت دیالیز صفاقی که این داروها را مصرف نمی‌کردند، سطح منیزیم با VLDL (Very low density lipoprotein) و تری گلیسیرید ارتباط داشت (001/0 > P). ارتباط بین سطح منیزیم سرم و کلسترول و همچنین VLDL، در بیماران تحت همودیالیز در زیر گروهی که آنتی لیپید مصرف نمی‌کردند، معنی‌دار بود (001/0 > P). در همین زیر گروه، سطح منیزیم با میزان تری گلیسیرید نیز ارتباط داشت (001/0 > P).نتیجه‌گیری: سطح سرمی منیزیم با پروفایل لیپید بیماران تحت دیالیز ارتباط و همبستگی دارد و اختلالات متابولیسم لیپیدها باعث ایجاد خطرات آترواسکلروز و بیماری‌‌های قلبی در این بیماران می‌گردد.

Background: Lipid abnormalities, is one of the major causes of cardiovascular diseases in under-dialysis patients. This study aimed to assess the relationship of serum magnesium and blood lipid profile in patients under the peritoneal dialysis and hemodialysis.Methods: This cross- sectional study was performed on 110 under-dialysis patients in Zahedan, Iran. Serum lipids and magnesium levels were measured. Data were analyzed using t and Pearson correlation tests.Findings: The mean age of the patients was 46.1 ± 14.7 years. In under-peritoneal dialysis patients, who were not under the treatment of antilipid agents, the correlation of serum magnesium with triglyceride and very low-density lipoprotein cholesterol (VLDL) was significant (P < 0.05); whereas the correlation of serum magnesium with cholesterol and low-density lipoprotein cholesterol (LDL-C) was significant only in subgroup who were under treatment of antilipid agents (P < 0.05). In under-hemodialysis group, the correlation of serum magnesium with cholesterol, triglyceride and  was significant (P < 0.05).Conclusion: This study showed that serum magnesium had correlation with serum lipid profile and could contribute to atherosclerosis and cardiovascular diseases in the patients under the peritoneal dialysis and hemodialysis.
