Background: Rupture of anterior crutiate ligament (ACL) is the most common severe injury of the knee. Following complete ACL rupture, insufficient revascularization prevents the complete healing of ACL as a result of which reconstruction methods are necessary. The appropriate treatment of this lesion remains a controversial issue. The most popular techniques of treatment are hamstring graft and bone-patellar tendon-bone graft. Therefore, the current study aimed to determine and compare the clinical results of ACL reconstruction on.Methods: This was a randomized prospective clinical trial involving 50 patients (20 to 35 years old), who had suffered from ACL tearing and had undergone surgery from December 2008 to December 2009. Known cases of psychological disease, functional disturbance diseases, and patients who had another medical condition or did not cooperate well in the follow-up period were excluded from the study. Patients were randomized into two equal groups. The first group was treated using bone-patellar-bone graft method and second group by hamstring graft method. Follow up examinations were performed at 2, 4, 6 and 9 months after operation. Data including the patient information, knee range of motion, rehabilitation time, anterior knee pain score, and antro-posterior and mediolateral plane translation were registered. Collected Data were analyzed by student’s t-test and Mann-Whitney test.Findings: 50 patients (49 men and 1 woman) with total mean age of 26.6 ± 4.5 (P value = 0.9) participated in the current study. There was a significant difference between the mean visual analogue scales of the two groups. , mean knee range of motion, antro-posterior and mediolateral plane translation scores, and rehabilitation time were not statistically significant during different times. Conclusion: In conclusion, since pain is a known post-oprative symptom, and due to the significantly low average pain in the hamstring group, it may be best to choose this method as the treatment method of anterior cruciate ligament reconstruction.
