مقایسه‌ی تأثیر دگزامتازون و لیدوکائین وریدی در پیش‌گیری از بروز اختلالات شناختی بعد از عمل جراحی کاتاراکت در سالمندان

چکیده

مقدمه: از بین اختلالات شناختی بعد از عمل جراحی به خصوص در سالمندان، دلیریوم از اهمیت بالاتری برخوردار می‌باشد. این اختلال می‌تواند باعث تغییراتی در شخصیت و حافظه‌ی بیمار شود و عوارض پس از عمل جراحی را افزایش دهد. در این مطالعه، تأثیر تجویز وریدی دگزامتازون و لیدوکائین بعد از القای بیهوشی عمومی در پیش‌گیری از بروز اختلالات شناختی بعد از عمل جراحی کاتاراکت در سالمندان مقایسه شد.روش‌ها: در این کارآزمایی بالینی آینده‌نگر شاهددار دو سو کور، 333 بیمار بالای 65 سال با شاخص American Society of Anesthesiologists (ASA) درجه‌ی I و II که کاندیدای عمل جراحی کاتاراکت به روش بیهوشی عمومی بودند، به صورت تصادفی به سه گروه لیدوکائین، دگزامتازون و شاهد (دارونما) تقسیم شدند و در هر بیمار، مقادیر نمره‌ی شناختی قبل از عمل و بعد از عمل ثبت شد.یافته‌ها: از سه گروه دریافت‌ کننده‌ی لیدوکائین، دگزامتازون و شاهد، 6/4،3/6 و 5/3 درصد قبل از عمل دچار اختلال شناختی بودند، اما بین سه گروه اختلاف معنی‌داری وجود نداشت (62/0 = P). تعداد افراد دچار اختلال شناختی در 24 ساعت بعد از عمل در سه گروه پیش‌گفته به ترتیب 0/12، 9/9 و 6/9 درصد بود و تفاوت بین سه گروه معنی‌دار نبود (98/0 = P). در یک هفته بعد از عمل نیز در سه گروه لیدوکائین، دگزامتازون و شاهد به ترتیب 3/8، 1/8 و 8/8 درصد دچار اختلال شناختی بودند، اما تفاوت سه گروه معنی‌دار نبود (98/0 = P).نتیجه‌گیری: در این مطالعه، استفاده از لیدوکائین و دگزامتازون در کاهش بروز اختلال شناختی، ارجحیتی نسبت به یکدیگر نداشت، اما از آن جایی که دگزامتازون در بیماران تحت عمل جراحی آب مروارید به علت وجود بیماری‌های زمینه‌ای محدودیت مصرف دارد، استفاده از لیدوکائین در این بیماران نسبت به دگزامتازون ارجحیت دارد.

A Comparison of the Effect of Intravenous Dexamethasone and Lidocaine on Prevention of Postoperative Cognitive Disorders in Cataract Surgery in Elderly Patients

چکیده [English]

Background: Postoperative cognitive disorder is one of the most common complications after surgery especially in the elderly population, of which delirium is the most important. This complication may cause changes in personality and memory of patients, and increase postoperative complications. In this study, we decided to compare the preventative effects of intravenous administration of dexamethasone and lidocaine on postoperative cognitive disorder after cataract surgery in the elderly.Methods: In this double-blind clinical trial study with control group, 333 patients above 65 years of age were divided randomly into three groups receiving lidocaine, dexamethasone, and placebo. The cognitive scores before and after the surgery were compared between the groups.Findings: In the three groups receiving lidocaine, dexamethasone, and placebo, 6.4%, 3.6%, and 5.3% had cognitive disorder prior to surgery, respectively, which had no significant difference (P = 0.62). The prevalence of the postoperative cognitive disorders in the three groups of lidocaine, dexamethasone, and placebo was 12.0%, 9.9%, and 6.9%, respectively, which was not significantly different either (P = 0.98). The prevalence of cognitive disorders after one week in the three groups of lidocaine, dexamethasone, and placebo was 3.8%, 1.8%, and 8.8%, respectively, which was not significantly different (P = 0.98).Conclusion: The administration of lidocaine or dexamethasone had no significant effect in reducing postoperative cognitive disorder, but since the use of dexamethasone is limited in patients with cataract undergoing surgery due to various underlying medical conditions, using lidocaine is preferable in such patients.
