بررسی مقایسه‌ای ویژگی‌های دو نوع صرع فراگیر و موضعی بر اساس تاریخچه‌ی خانوادگی صرع

چکیده

مقدمه: در اتیولوژی صرع، عوامل مختلفی مطرح است که یکی از این عوامل، ژنتیک می‌باشد. هدف از این مطالعه، بررسی مقایسه‌ای تأثیر تاریخچه‌ی خانوادگی مثبت در دو نوع صرع فراگیر و موضعی و مقایسه‌ی تفاوت‌های ویژگی‌های بیماران بر اساس تاریخچه‌ی خانوادگی بود.روش‌ها: مطالعه‌ی حاضر، یک مطالعه‌ی توصیفی- تحلیلی گذشته‌نگر و مقطعی بود. اطلاعات بیماران (با تشخیص قطعی صرع توسط متخصص مغز و اعصاب) مراجعه کننده به کلینیک صرع بیمارستان‌های آیت‌اله کاشانی و الزهرای (س) اصفهان طی سال‌های 92-1388 وارد مطالعه گردید. با استفاده از پرسش‌نامه، اطلاعات دموگرافیک بیماران و تاریخچه‌ی خانوادگی آن‌ها از نظر وجود بیماری صرع در خانواده، درجه‌ی یک و دو تعیین شد.یافته‌ها: رابطه‌ی مثبتی بین صرع موضعی و تاریخچه‌ی خانوادگی مثبت صرع در بیماران مبتلا وجود داشت (046/0 = P)؛ چنین رابطه‌ی مثبتی در بیماران مبتلا به صرع عمومی مشاهده نشد (098/0 = P). در بیماران مبتلا به هر دو نوع صرع، اکثریت معنی‌داری از افراد با الکتروانسفالوگرافی (Electroencephalography یا EEG) و CT scan (Computed tomography scan) غیرطبیعی مغز بدون سابقه‌ی خانوادگی بودند. بر حسب جنسیت، وضعیت تأهل و دارا بودن تحصیلات آکادمیک، اکثر افراد سابقه‌ی خانوادگی نداشتند (001/0 > P)؛ اما مدت تشنج بیماران مبتلا به صرع موضعی برای افرادی که سابقه‌ی خانوادگی درجه‌ی دو داشتند، بالاتر از سایرین بود.نتیجه‌گیری: با توجه به وجود ارتباط معنی‌دار بین صرع موضعی و تاریخچه‌ی خانوادگی صرع، توصیه می‌شود مطالعات بیشتری هم در سطح مولکولی و هم در سطح اپیدمیولوژیک در حجم‌های وسیع‌تر انجام شود که نقش عوامل ژنتیکی در بروز این بیماری نمایان‌تر گردد.

Background: In etiology of epilepsy, different factors have role and one of them is genetics. This study aimed to investigate the differences between generalized and partial epilepsy based on the family history.Methods: The patients with epilepy admitted in the epilepsy clinics of Kashani and Alzahra Hospitals, Isfahan, Iran, during the years 2009-2013, enrolled in this cross-sectional retrospective study. Demographic data of these patients were gathered with a questionnaire form. First and secondary degree family history of patients was determined. The epilepsy was diagnosed by an experienced neurologist using history and physical exam and was confirmed with brain computed tomography (CT) scan and electroencephalogram (EEG).Findings: There was a positive correlation between the partial epilepsy and positive family history of epilepsy (P = 0.046) but not in generalized epilepsy (P = 0.098). There was significant correlation between abnormal brain CT scan, abnormal EEG, sex, marriage and education with negative family history of epilepsy in both generalized and partial epilepsy; although, the duration of seizure in patients with partial epilepsy who had secondary degree positive family history was significantly more than the others (P < 0.001).Conclusion: According to positive correlation between partial epilepsy and family history of epilepsy, more molecular and epidemiological researches would be recommended to be be arranged for determination of the the role of genetic factors in epilepsy.
