مقایسه‌ی تأثیر ترکیب سه داروی دگزامتازون،کتامین و فنتانیل با متوکلوپرامید، کتامین و فنتانیل بر درد بعد از عمل بیماران کاندید کوله سیستکتومی لاپاراسکوپیک

چکیده

مقدمه: درد بعد از عمل کوله‌سیستکتومی لاپاراسکوپیک، واکنش فیزیولوژیک پیچیده‌ای است که می‌تواند با عوارض شدید در ارگان‌های مختلف مرتبط باشد. از این‌رو، هدف از انجام مطالعه‌ی حاضر، مقایسه‌ی اثرات ترکیب دگزامتازون، فنتانیل و کتامین با ترکیب متوکلوپرامید، فنتانیل و کتامین بر درد بعد از عمل در بیماران کاندید کوله‌سیستکتومی لاپاراسکوپیک بود.روش‌ها: در این پژوهش کارآزمایی بالینی دو سوکور تصادفی، 96 بیمار کاندید عمل کوله‌سیستکتومی لاپاراسکوپیک با طیف سنی 18 تا 80 سال، به طور تصادفی در سه گروه قرار گرفتند. گروه اول، 15/0 میلی‌گرم بر کیلوگرم دگزامتازون، 3/0 میلی‌گرم برکیلوگرم کتامین و 5/0 میکروگرم بر کیلوگرم فنتانیل، گروه دوم، 10 میلی‌گرم متوکلوپرامید، 3/0 میلی‌گرم برکیلوگرم کتامین و 5/0 میکروگرم بر کیلوگرم فنتانیل و گروه سوم تنها داروهای القای بیهوشی دریافت کرد. میزان درد و تهوع با استفاده از معیار Visual analog scale (VAS) و همچنین، تعداد دفعات استفراغ و میزان ماده‌ی ضد تهوع و مخدر دریافتی در 2، 6، 12 و 24 ساعت بعد از عمل جراحی اندازه‌گیری گردید.یافته‌ها: میانگین امتیاز درد در 2، 6، 12 و 24 ساعت بعد از جراحی به طور معنی‌داری در گروه اول کمتر از سایر گروه‌ها بود (050/0 > P). میزان مخدر مصرفی بعد از عمل 2 و 6 ساعت بعد از جراحی در گروه اول به طور معنی‌داری کمتر از سایر گروه‌ها بود (050/0 > P). میانگین امتیاز تهوع و مصرف داروی ضد تهوع در گروه دوم به طور معنی‌داری کمتر از سایر گروه‌ها بود (050/0 > P).نتیجه‌گیری: ترکیب سه داروی دگزامتازون، کتامین و فنتانیل به طور معنی‌داری درد بعد از عمل را در بیماران کاندید عمل کوله‌سیستکتومی لاپارسکوپیک کاهش می‌دهد. گرچه این تأثیر فقط به طور نسبی با ترکیب سه داروی متوکلوپرامید، کتامین و فنتانیل به دست آمد. هر دو ترکیب دارویی به طور معنی‌داری منجر به کاهش تهوع و استفراغ بعد از عمل جراحی می‌شود.

Background: Pain after laparoscopic cholecystectomy is a complex physiologic reaction which can be associated with severe complications in various organs. In this study, we aimed to compare the effects of combination of dexamethasone, fentanyl, and ketamine with combination of metoclopramide, fentanyl, and ketamine on postoperative pain in patients undergoing laparoscopic cholecystectomy.Methods: In this randomized, double-blind clinical trial study, 96 patients undergoing laparoscopic cholecystectomy aged 18-80 years were randomly allocated to three groups. Group 1 received dexamethasone (0.15 mg/kg), ketamine (0.3 mg/kg), and fentanyl (0.5 µg/kg); group 2 received metoclopramide (10 mg), ketamine (0.3 mg/kg), and fentanyl (0.5 µg/kg); and group 3 received only anesthetic induction drugs. The amount of pain and nausea were measured using the visual analog scale (VAS), as well as the frequency of vomiting and the amount of anti-nausea and drug administered at 2, 6, 12, and 24 hours after surgery.Findings: Mean pain score at 2, 6, 12, and 24 hours after surgery was significantly lower in group 1 than other groups (P < 0.05). Postoperative consumption of opioid at 2 and 6 hours after surgery was significantly lower in group 1 than other groups (P < 0.05). The mean score of nausea and anti-emetic consumption were significantly lower in group 2 than other groups (P < 0.05).Conclusion: The combination of three drugs, dexamethasone, ketamine, and fentanyl significantly reduced postoperative pain in patients undergoing laparoscopic cholecystectomy, whereas this effect was only partially attained with the combination of three drugs, metoclopramide, ketamine, and fentanyl. Both combinations of the drugs significantly reduced postoperative nausea and vomiting.
