بررسی میزان افزایش پلاکت به دنبال تزریق یک واحد پلاکت در بیماران بستری در بخش مراقبت‌های ویژه

چکیده

مقدمه: ترانسفیوژن پلاکت به صورت شایع در بیمارانی که وضعیت بحرانی دارند و همچنین، در برخی موارد در بخش مراقبت‌های ویژه (Intensive care unit یا ICU)، برای جلوگیری از ایجاد ترومبوسیتوپنی انجام می‌گیرد. هدف از انجام این مطالعه، بررسی میزان افزایش پلاکت به دنبال تزریق یک واحد پلاکت در بیماران بستری در ICU بود.روش‌ها: در این مطالعه‌ی گذشته‌نگر، تعداد 130 بیمار که در ICU یک واحد پلاکت دریافت کرده بودند، با توجه به معیارهای ورود و خروج مطالعه انتخاب شد. اطلاعات بیماران در یک چک لیست ثبت شد و میزان افزایش پلاکت بیماران در طی 7 روز از بستری با اطلاعات به دست آمده مقایسه گردید.یافته‌ها: میانگین میزان افزایش پلاکت در بیماران، 103 × (35/10 ± 16/31) میکرولیتر بود و بین میزان افزایش پلاکت با طول مدت بستری در ICU و تعداد دفعات خون‌گیری همبستگی معنی‌دار معکوسی وجود داشت (050/0 > P).نتیجه‌گیری: ترانسفیوژن پلاکت در بیماران بستری در بخش مراقبت‌های ویژه، باعث کوتاه شدت طول مدت بستری دراین بخش و همچنین، افزایش تعداد دفعات خون‌گیری باعث کاهش افزایش میزان پلاکت در این بیماران می‌شود.

Background: Platelet transfusion is used in patients with a critical condition, and in some cases in the intensive care unit (ICU) to prevent thrombocytopenia. The aim of this study was to evaluate platelet elevation following injection of a platelet unit in patients admitted to intensive care unit.Methods: In this retrospective study, 130 patients who received a unit of platelet in the intensive care units of Alzahra hospital, Isfahan, Iran, were selected according to the inclusion and exclusion criteria of the study. Patients' information were recorded in the checklist, and the rate of platelet elevation in patients within 7 days of admission was compared with the data obtained.Findings: The mean elevation of platelet count in patients was 31.16 ± 10.35 × 103/μl. There was a reverse significant correlation between the elevation of platelet count with the duration of hospitalization in intensive care unit and the number of blood sampling times (P < 0.05).Conclusion: Platelet transfusion in patients admitted to intensive care unit may cause short duration of hospitalization; in addition, increase in the number of blood sampling times leads to a decrease in platelet count in these patients.
