مقدمه: مولتیپل اسکلروزیس (Multiple Sclerosis) MS، یک بیماری خودایمنی مزمن سیستم عصبی مرکزی است که ارتباط شاخص‌های آزمایش شمارش کامل خون (Complete Blood Count) CBC با آن به‌طور کامل بررسی نشده است. این مطالعه با هدف ارزیابی ارتباط بین یافته‌های مختلف آزمایش شمارش کامل خون با ویژگی‌های جمعیت‌شناختی و بالینی بیماران مولتیپل اسکلروزیس و مقایسه با گروه کنترل انجام شد.
روش‌ها: این مطالعه‌ی مقطعی با مشارکت 1764 نفر ۷۲۹ بیمار MS و ۱۰۳۵ گروه کنترل در کلینیک تخصصی بیمارستان کاشانی انجام شد. نمونه‌های خون برای تحلیل CBC جمع‌آوری و داده‌های دموگرافیک و بالینی ثبت شدند.
یافته‌ها: تمام شاخص‌های آزمایش شمارش کامل خون CBC به جز گلبول‌های قرمز خون (Red Blood Cells) RBC و هماتوکریت (Hematocrit) HCT با مولتیپل اسکلروزیس ارتباط معنی‌داری داشتند. ما سطوح بالاتری از هموگلوبین (Hemoglobin) HB (0/001 > P، 1/407 = PEst) هماتوکریت (HCT) (0/001 > P، 3/937 = Est)، حجم متوسط گویچه‌ای (Mean Corpuscular Volume) MCV (0/042 = P، 1/482 = Est) و میانگین غلظت هموگلوبین (Mean Corpuscular Hemoglobin Concentration) MCHC (0/011 > P، 0/443 = Est) را در بیماران مذکر مشاهده نمودیم. نتایج آزمایش‌های بیمارانی که از چندابتلایی‌های خودایمنی رنج می‌بردند در مقایسه با بیمارانی که چندابتلایی‌های خودایمنی نداشتند، سطوح پایین‌تری از گلبول‌های قرمز خون (RBC) (0/001 > P، 7/440 - = Est) و هموگلوبین (0/001 > P، 2/610 - = Est)، را نشان دادند. هموگلوبین (0/025 = P، 1/447 = Est)، هماتوکریت (0/002 = P، 4/877 = Est) و حجم متوسط گویچه‌ای (0/027 = P، 5/670 = Est)، در بیماران مولتیپل اسکلروزیس‌ پیش‌رونده‌ی اولیه (PPMS) از بیماران مولتیپل اسکلروزیس‌ عودکننده‌بهبودیابنده (RRMS) بیش‌تر بودند. بین نخستین بررسی از «مقیاس گسترده‌ی وضعیت ناتوانی (EDSS)» و شاخص‌های آزمایش شمارش کامل خون (CBC) هیچ ارتباطی وجود نداشت.
نتیجه‌گیری: یافته‌های مطالعه می‌توانند به درک بهتر نقش التهاب سیستمیک در پاتوفیزیولوژی MS کمک کنند، هرچند برای تعیین ارزش تشخیصی یا پیش‌آگهی CBC در MS به پژوهش‌های بیشتری نیاز است.

Background: Multiple sclerosis (MS) is a chronic, inflammatory, immune-mediated disease of the central nervous system (CNS). Although complete blood count (CBC) is known a marker of systematic inflammation, relation between CBC indexes such and MS remains understudied. This study was conducted to assess the association between different CBC findings with demographic and clinical features of MS patients and compare them with healthy control (HC).
Methods: This cross-sectional study was conducted in MS clinic of Kashani Hospital. A total of 1764 participants were involved in this study, including 729 (41.3%) patients with MS and 1035 (58.7%) control was enrolled. CBC counts were obtained via routine blood sampling, as part of the clinical practice. HC recruited from people who visited our Hospital for routine checks-up.
Findings: All CBC indexes were significantly associated with MS except for RBC and HCT. We observed higher HB (Est. = 1.407; P < 0.001), HCT (Est. = 3.937; P < 0.001), MCV (Est.=1.482; P = 0.042), MCHC (Est.=0.443; P = 0.011) in male. Patients with autoimmune comorbidity demonstrated less RBC (Est. = -7.440; P < 0.001) and HB (Est. = -2.610; P < 0.001) compared to patients without autoimmune comorbidity. HB (Est. = 1.447; P = 0.025), HCT (Est. = 4.877; P = 0.002), MCV (Est. = 5.670; P = 0.027) were higher in PPMS than RRMS. There was no association between the first EDSS and CBC indexes.
Conclusion: Our results showed an association between CBC markers and MS. We found no association between blood indexes and clinical findings.
