Abstract
Spondylodiscitis is a severe spinal infection that poses diagnostic and therapeutic challenges. The Surgical Infection Treatment and Severity (SITE) scoring system was developed to support surgical decision-making in infectious spinal diseases. This study aimed to assess the clinical applicability and supportive validity of the SITE scoring system in patients with de novo pyogenic spondylodiscitis. Methods: This retrospective analytical study included 165 adult patients with confirmed de novo spondylodiscitis who were managed either surgically or conservatively according to SITE-based criteria. Patients were divided into two severity groups (SITE < 8 and SITE ≥ 8). Clinical and radiological outcomes were assessed, and recovery was categorized as partial, full, or second full recovery. Sensitivity, specificity, and optimal cut-off values for predicting surgical need were determined using ROC curve analysis and Youden’s index. lskdflkesd Results: Among 165 patients (mean age 54.6 ± 14.7 years; 52% male), 61.2% underwent surgery. The SITE score showed acceptable association
