Multiple Approaches to Promoting Physical Activity in Society

Abstract
Background: Physical inactivity, along with overweight, is a risk factor for the highest burden of disease in Iran. This article aims to explain the multiple approaches required for promoting physical activity (PA) in Iranian citizens.
Methods: This study was conducted as a narrative review study that conducted by reviewing reference books and articles published until 2025 in the field of public health and lifestyle modification in scientific databases such as ISC, IranDoc, Google Scholar, PubMed, Science Direct.
Findings: Promoting PA requires a multifaceted approach. Policymaking for sustainable financing to support the PA package through budget allocation to government comprehensive health centers, as well as the purchasing of the PA package by insurance organizations, can increase the utilization rate of the PA service package delivered by experienced experts among patients (especially those with multiple conditions) or overweight individuals with socioeconomic limitations, either within government comprehensive health centers or private sports clubs. In addition, empowering the primary health care team in PA counseling, and performing examination, screening, and delivering PA counseling services through a multidisciplinary team with a socioecological approach, utilizing the 5A steps ("Assess, Advise, Agree, Assist, and Arrange"), can increase citizens' adherence to PA counseling services.
Conclusion: Integrating a PA service package into the primary health care delivery system can substantially promote the PA level in society.
