Therapeutic Outcomes of Using a Simple External Fixator in the Treatment of Finger Fracture

Abstract
Background and Objective: Complex finger fractures, particularly intra-articular and open types, are among the most challenging hand injuries due to the high risk of nonunion, malunion, and functional impairment. The use of a simple external fixator, as a cost-effective and readily available technique, provides skeletal stability while enabling early joint mobilization. This study aimed to evaluate the clinical and functional outcomes of this method in the treatment of complex finger fractures.
Methods: In a prospective descriptive-analytical study, 25 patients with complex finger fractures (including open and intra-articular fractures) were treated using a simple external fixator. Patients were evaluated for bone union time, occurrence of complications (superficial and deep infection, nonunion, malunion), and functional outcomes using the Total Active Motion (TAM) criteria.
Results: All patients achieved satisfactory bone union, with no cases of nonunion or malunion. The mean union time was 3.9±1.4 weeks. Superficial pin-tract infection occurred in 3 patients (12%) and was controlled with medical therapy; no deep infections or re-operations were reported. Based on TAM assessment, 84% of patients achieved good or excellent outcomes.
Conclusion: The simple external fixator is a safe, cost-effective, and effective option for the management of complex finger fractures, including intra-articular and open injuries. It provides stable fixation, satisfactory bone healing, and functional recovery. This technique can be considered a suitable alternative to expensive commercial fixators, particularly in resource-limited settings.
