بررسی و مقایسه‌ی تحمل دارویی و نتایج درمانی ناشی از تجویز آموکسی سیلین و سفیکسیم در اوتیت میانی حاد

چکیده

مقدمه: درمان صحیح و به موقع اوتیت میانی حاد می‌تواند از عوارض ناشی از بیماری جلوگیری کند. اساس درمان اوتیت میانی حاد تجویز آنتی بیوتیک است. از آن جایی که شیوع مقاومت میکروبی در مقابل آنتی بیوتیک‌ها در جوامع مختلف متفاوت است، در سال‌های اخیر تصمیم گیری پزشکان در امر درمان بر اساس نتایج حاصل از تحقیقات انجام شده رو به افزایش است. این مطالعه با هدف تعیین و مقایسه‌ی تحمل دارویی و نتایج درمانی ناشی از تجویز آموکسی سیلین و سفیکسیم در اوتیت میانی حاد انجام شد.روش‌ها: در یک کارآزمایی بالینی، 200 کودک با سابقه‌ی 2 سال ابتلا به اوتیت میانی حاد شرکت داده شدند. نمونه‌ها به روش آسان تداومی انتخاب و سپس به طور تصادفی به دو گروه دریافت کننده‌ی آموکسی سیلین و سفیکسیم تقسیم شدند. اطلاعات از طریق مصاحبه، مشاهده و تیمپانومتری جمع‌آوری شد. تجزیه و تحلیل اطلاعات با استفاده از روش‌های آماری توصیفی و استنباطی با استفاده از نرم‌افزار SPSS انجام شد.یافته‌ها: فراوانی نشانه‌های مربوط به اوتیت میانی در دو گروه بعد از اتمام مصرف آنتی‌بیوتیک تفاوت معنی‌دار نداشت. به عبارت دیگر، فراوانی بهبودی کامل اوتیت میانی حاد در پایان درمان دارویی در دو گروه یکسان بود. از نظر تحمل دارویی در هیچکدام از کودکان مورد مطالعه، عارضه‌ای مشاهده نشد و تنها در گروه آموکسی سیلین یک کودک دچار اسهال خفیف گردید. نتیجه‌گیری: اثربخشی و تحمل دارویی دو داروی آموکسی‌سیلین و سفیکسیم یکسان است و از آن جایی که سفیکسیم دارویی است که می‌تواند با تعداد دفعات کمتر در روز در مقایسه با آموکسی سیلین مصرف شود، می‌توان در مواردی که کودک با مصرف داروهای خوراکی ناسازگاری دارد، در اوتیت میانی حاد به جای آموکسی سیلین از سفیکسیم استفاده کرد.

Background: Proper and on time treatment of acute otitis media could prevent complications. Antibiotic therapy has the principal role in treatment. In recent years, modalities of treatment are increasingly become evidence based. This study was done to determine and compare the efficacy and therapeutic tolerance of amoxicillin and cefixime in children with acute otitis media.Methods: In a randomized clinical trial, 200 more than 2 years children with acute otitis media were enrolled by simple convenient method. They were randomly divided in two groups, treated with amoxicillin or cefixime. Data were gathered by interview, observation, and tympanometery testand were analyzed via SPSS software.          Finding: The frequency of signs of acute otitis media after completing of drug therapy was not significantly different between the two groups. In the other words, the cure rate in two groups was equal. In regard to drug tolerance, there was not any significant adverse drug reaction in two groups but, only one patient among the amoxicillin group suffered from mild diarrhea.Conclusion: Based on findings of the study, we can say that both drugs have the same efficacy and tolerability. So in children have difficulty with oral drugs coping, an alternative antibiotic with longer duration of half life such as cefixime can be the first line drug for treatment of acute otitis media.
