بیمار با شکایات جسمی بدون علت طبی مشخص

چکیده
فراگیر در پایان مطالعه این خودآموز باید بتواند:1- آگاهی‌های لازم در مورد شکایت جسمی بدون  علت طبی را کسب کند.2- از اقدامات تشخیصی و درمانهای غیرضروری بیماران اجتناب کند.3- رویکردهای مناسب برخورد با این بیماران را فرا گیرد.4- موارد مناسب ارجاع به رشته‌های مختلف طبی را تشخیص دهد. چکیدهشکایت‌های جسمی غیر قابل توضیح طبی (Medically unexplained symptoms  یا MUS) در مراجعین به مطب‌ها و کلینیک‌های مختلف شیوع به نسبت زیادی دارد. در بعضی مطالعات، حدود نیمی از مراجعین به پزشکان عمومی را مبتلا به درجاتی از این مشکل دانسته‌اند. مراجعات مکرر، اقدامات تشخیصی و درمانی فراوان و در نتیجه هزینه‌های به نسبت زیاد باعث فشار بر بیماران، خانواده‌ی آن‌ها و سیستم‌های بهداشتی-درمانی می‌شود. از طرفی تداوم شکایت‌های جسمی این گروه از بیماران و ناتوانی پزشکان معالج در توجیه این شکایات با وجود بررسی‌ها و اقدامات تشخیصی، موجب بروز مشکلات عدیده بالاخص در رابطه‌ی درمانی بین پزشک و بیمار می‌گردد.

چکیده English

Medically unexplained symptoms (MUS) are symptoms that are not explained by an organic disease. MUS are common in all medical settings; as many as 50% of patients who visit the general practitioners have some degree of these symptoms. Unexplained symptoms can cause significant distress for the patients and their families, and in some circumstances impair functioning. Continuation of symptoms and resistance to routine treatments may cause significant distress and disrupt the physician-patient relationship.
