چکیده
مقدمه: کودکان 70-30 درصد بیماران مراجعهکننده به درمانگاههای سرپایی را تشکیل میدهند. مطالعات مختلف نشان داده است که برنامهی مراقبتهای ادغامیافتهی ناخوشیهای اطفال (مانا) باعث بهبود کیفیت مراقبتهای سرپایی در کودکان میشود. هدف مطالعهی حاضر، مقایسهی عملکرد کارورزان در مراقبتهای سرپایی کودکان زیر 5 سال در دو روش مراقبت معمول و مراقبتهای ادغامیافتهی ناخوشیهای اطفال (مانا) در درمانگاه تخصصی کودکان بود. روشها: تحقیق حاضر به صورت نیمه تجربی و دو سو کور در بیمارستان امین اصفهان صورت گرفت. کلیهی کارورزان حاضر در دو درمانگاه کودکان در طی مدت مطالعه با روش سرشماری انتخاب شدند. عملکرد کارورزان مذکور در نحوهی ویزیت کودکان مراجعهکننده توسط یک مشاهدهگر و از طریق چک لیست ارزیابی گردید. در مرحلهی بعد کارورزان یکی از درمانگاهها در مورد راهنماهای بالینی مانا آموزش دیدند. در پایان دوره عملکرد کارورزان در نحوهی ویزیت کودکان در دو گروه ارزیابی مجدد شد. تجزیه و تحلیل دادهها توسط آزمونهای Student-t، Paired-t و 2χ انجام گردید. یافتهها: میانگین نمرهی عملکرد کارورزان در دو درمانگاه در پایان دوره افزایش یافته بود و در دو گروه در پایان دوره تفاوت معنیداری دیده نشد (05/0 < P). فراوانی نسبی فقط در چند مورد از موارد چک لیست در دو گروه تفاوت معنیداری داشت. نتیجهگیری: عدم مشاهدهی تفاوت قابل ملاحظه ممکن است به دلیل کم بودن مدت زمان آموزش، کوتاه بودن زمان پیگیری، علاقهی بیشتر کارورزان به کتب مرجع و عدم فرصت کافی کارورزان در استفاده از آموختهها باشد. واژگان کلیدی: کودکان قبل از مدرسه، کیفیت مراقبت سلامت، مراقبتهای ادغامیافتهی ناخوشیهای اطفال (مانا)

چکیده English
Background: Children account for 30-70% of all patients seen in outpatient clinics. Studies have shown integrated methods to improve quality of outpatient care in children. The present study aimed to compare quality of care between routine and integrated methods in pediatric outpatient setting. Methods: This double-blinded prospective study was carried out in two pediatric clinics of Amin Hospital (Isfahan, Iran). Non-probability convenience sampling was used to select 100 children. Performance of medical interns in visiting these children was assessed by an observer through a checklist. Then, students in clinic A were trained in Integrated Management of Childhood Illness (IMCI) algorithms. The performance of the two groups in visiting 100 children was reevaluated at the end of the period again. The mean of their scores and frequency of each item of the checklist was calculated and compared between the two groups. Findings: The mean scores of performance of the two groups had no significant difference. Only the frequency of few items of the checklist was significantly different between the two groups. Conclusion: We found no significant difference between the two groups. It may be due to short training period, interest in textbooks, lack of knowledge about efficacy of IMCI algorithms in the future work, and not having adequate time for practicing.
