بررسی میزان تأثیر پارامترهای اسپرمی بر روی نتایج درمان به روش تزریق درون رحمی اسپرم

چکیده

مقدمه: روش تزریق درون رحمی اسپرم (Intrauterine insemination یا IUI) یکی از شایع‌ترین روش‌های مورد استفاده در باروری زنان است که خصوصیات اسپرمی در آن دخیل هستند. مطالعه‌ی حاضر نیز با هدف تعیین میزان تأثیر پارامترهای اسپرمی بر روی نتایج درمان به روش IUI به انجام رسید.روش‌ها: این مطالعه‌ی توصیفی- تحلیلی است در سال 1391 در بیمارستان شهید بهشتی اصفهان انجام شد. جمعیت مورد مطالعه، نمونه‌های مایع منی افرادی بود که به مرکز ناباروری شهید بهشتی اصفهان، جهت درمان به روش IUI مراجعه نموده بودند. این نمونه‌ها به صورت Simple sampling جمع‌آوری گردیدند. در طی این بررسی، پارامترهای دانسیته‌ی اسپرمی، درصد حرکت چهارگانه‌ی اسپرمی، درصد مورفولوژی طبیعی اسپرمی و میزان ویسکوزیتی مایع منی و تجمع (naggregatiA) مورد ارزیابی قرار گرفت. داده‌های به دست‌آمده پس از ورود به رایانه به وسیله‌ی نرم‌افزار SPSS مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: در این مطالعه پارامترهای اسپرمی در 80 نمونه که تحت درمان ناباروری به روش IUI قرار گرفته بودند، بررسی شد. از بین 80 بیماری که تحت درمان با این روش قرار گرفتند، 16 نفر باردار شدند و روش مذکور در 64 نفر موفقیت‌آمیز نبود. میانگین حجم مایع سمن، غظت مایع و پارامترهای حرکتی اسپرم در دو گروه باردارشده و باردارنشده اختلاف معنی‌داری نداشت.نتیجه‌گیری: مطابق نتایج این مطالعه علاوه بر عوامل اسپرمی، فاکتورهای دیگری مثل تکنیک تزریق اسپرم، مراقبت پزشکی، عوامل ثانوی مانند استعمال سیگار و تجهیزات مورد استفاده در نگه‌داری و تزریق اسپرم نیز امی‌توانند در موفقیت باروری روش IUI تأثیرگذار باشند. به همین دلیل پیشنهاد می‌گردد ضمن بازنگری پروتکل‌های جاری باروری به روش IUI و تطبیق آن با استانداردهای بین‌المللی، زنان کاندید روش مذکور، به همراه همسران خود قبل از تزریق اسپرم، به طور کامل تحت آموزش قرار گیرند.

Background: Infertility is common among couples of childbearing age. Approximately, half of known causes of primary infertility are attributable to male factor and the etiology of male factors infertility is poorly understood. While some individuals may be genetically predisposed to be sub-fertile, there are major epigenetic factors implicated as potential causes of male infertility. Semen characteristics such as morphology, concentration and motility have positive effect of infertility. One of the most prevalent methods for solving this problem is Intra-Uterine Insemination (IUI) and the aim of this study was determining the effect semen characteristics on IUI outcomes.Methods: This cross sectional study was done in Shahid Beheshti hospital (Isfahan, Iran) during March to September 2012. 80 samples of semen of infertile men were selected and analyzed by semen analysis and semen processing methods. Semen characteristics such as physical characteristics and motility were recorded and analyzed.Findings: Fertility in 16 cases (20%) was success and in 64 (80%) was unsuccess. Semen characteristics including volume, concentration and motility was not statistically different between these two groups.Conclusion: According to results of this study, except semen characteristics, some other factors such as technique of sperm injection, medical instruments and patient factors may have a role in unfertility. Thus, this factors and related protocols mast be revised and couple education mast be done before sperm injection.
