آسیب‌های عروقی در نفریت لوپوسی: نیازی برای برای توجه بیشتر به واسکولوپاتی در این بیماران

چکیده
آسیب‌های عروقی از مهم‌ترین عوامل در پاتوژنز و پیشرفت بسیاری از بیماری‌های اولیه و ثانویه‌ی کلیه به ویژه گلومرولوپاتی‌ها می‌باشند (2-1). در واقع، بسیاری از بیماری‌های کلیوی از ضایعات اولیه‌ی عروقی ناشی می‌شوند، اما در اکثریت موارد، در بیماری‌هایی که به طور اولیه اجزای گلومرول‌ها یا توبولواینترستیشیال را تحت تأثیر قرار می‌دهند، ساختارهای عروقی به طور ثانویه درگیر می‌شوند. نفریت لوپوسی و IgA نفروپاتی (IgA nephropathy یا Immunoglobulin nephropathy) از بهترین مثال‌های بیماری‌هایی هستند که بر روی بخش‌های مختلف بافت کلیه شامل گلومرول‌ها، رگ‌ها، بافت بینابینی و توبول‌ها اثر می‌گذارند (5-2). در این بیمار‌ی‌ها، علاوه بر ضایعات پارانشیم کلیه، آسیب‌های متعدد عروق کلیه که با مکانیسم‌های متفاوتی ایجاد شده‌اند، دیده می‌شود. تشخیص و طبقه‌بندی دقیق آسیب‌های عروقی، نقشی مهم در کنترل ایده‌آل این بیماری‌ها ایفا می‌کند (6-5)...

چکیده English

Vascular lesions are of significant importance in the progression of various kidney diseases. Indeed, various kidney diseases result from primary lesions to the vascular component; however in some cases, vascular compartment is involved secondarily in the disease process affecting primarily either the glomeruli or the tubulointerstitial components. Recently, much attention was made toward vasculopathy in patients with lupus nephropathy. Most of these morphologic lesions belong to antiphospholipid antibody syndrome nephropathy. In this regard, it is necessary to precisely define vascular lesions in patients with lupus nephropathy.
