چکیده
مقدمه: استراتژیهای کنونی درمان پسوریازیس، رضایتبخشی کاملی ایجاد نمیکند. با مهار سایتوکینهای التهابی، نیکوتین آمید میتواند تأثیر درمانهای موضعی کنونی را افزایش دهد. در این مطالعه، اثر بخشی ترکیب کلسیپوتریول موضعی و نیکوتین آمید در مقایسه با کلسیپوتریول بهتنهایی، در درمان پسوریازیس مورد بررسی قرار گرفت.روشها: بیماران بزرگسال مبتلا به پسوریازیس خفیف تا متوسط به صورت تصادفی انتخاب شدند وترکیب کلسیپوتریول 005/0 درصد و نیکوتین آمید 4 درصد، یا کلسیپوتریول 005/0 درصد بهتنهایی را دو بار در روز به مدت 12 هفته به صورت موضعی دریافت کردند. بیماران در شروع مطالعه و سپس پس از ماه اول و سوم درمان، توسط یک متخصص پوست معاینه شدند و شدت پسوریازیس با استفاده از منطقهی اصلاح شدهی پسوریازیس و شاخص شدت پسوریازیس (PASI یا Psoriasis area and severity index) مورد بررسی قرار گرفت. رضایت بیمار نیز در انتهای آزمایش با استفاده از یک مقیاس رتبهبندی 10 نقطهای بررسی شد.یافتهها: شصت و پنج بیمار (شامل 35 مرد) با میانگین سنی 5/8 ± 5/36 سال در مطالعه شرکت کردند. در شروع مطالعه، ضایعات هر دو گروه با توجه به شاخص PASI مشابه بود. در پایان آزمایش، نمرهی PASI با کلسیپوتریول و نیکوتین آمید (9/7 ± 6/83 درصد)، در مقایسه با کلسیپوتریول تنها (7/9 ± 8/77 درصد)، بیشتر کاهش یافت (001/0 > P). همچنین، بیماران بهبود یافته در گروه کلسیپوتریول و نیکوتین آمید در مقایسه با کلسیپوتریول تنها، رضایت بیشتری داشتند (001/0 > P). عوارض جانبی شامل قرمزی و خارش خفیف (6/4 درصد) و سوزش متوسط و حساسیت به نور (0/3 درصد) بود.نتیجهگیری: نیکتوین آمید در صورتی که همراه با کلسیپوتریول در درمان موضعی پسوریازیس به کار برده شود، اثر بخشی آن را افزایش میدهد و میتواند مکمل خوبی برای درمان فعلی پسوریازیس محسوب گردد.

چکیده English
Background: Current treatment strategies of psoriasis are not completely satisfactorily. By inhibiting inflammatory cytokines, nicotinamide may enhance the effects of current topical treatments. We investigated whether the combination of topical calcipotriol and nicotinamide is more effective than calcipotriol alone in treatment of psoriasis.Methods: Adult patients with mild to moderate psoriasis were randomized to receive topical calcipotriol 0.005% and nicotinamide 4% in combination or calcipotriol 0.005% alone, twice daily for 12 weeks. Patients were visited by a dermatologist at baseline and then, after the first and third months of therapy; and psoriasis severity was evaluated using the modified psoriasis area and severity index (PASI). Also, patient’s satisfaction was evaluated at the end of the trial using a 10-point rating scale.Findings: Sixty five patients (35 men) with the mean age of 36.5 ± 8.5 years completed the trial. Lesions on both sides were similar regarding baseline PASI score. At the end of the trial, PASI score was more reduced with calcipotriol + nicotinamide (83.6 ± 7.9 percent) compared to calcipotriol alone (77.8 ± 9.7 percent) (P < 0.001). Patients were also more satisfied with the improvement of lesions with calcipotriol + nicotinamide compared to calcipotriol alone (P < 0.001). Side effects included mild erythema and pruritus (4.6%) and moderate burning and sensitivity to light (3.0%).Conclusion: Nicotinamide can enhance the efficacy of calcipotriol when used in combination for topical psoriasis treatment, and it may be a good adjuvant to the current treatment regimens of psoriasis.
