بررسی تأثیر سوسپانسیون سلولی ملانوسیت در محیط Jokliks در درمان ویتیلیگوی ثابت مقاوم: گزارش 10 مورد

چکیده
مقدمه: ویتیلیگو یک اختلال دپیگمانته‌ی اکتسابی ایدیوپاتیک است که به دلیل کاهش فعالیت ملانوسیت‌های اپی‌درم ایجاد می‌شود. روش‌های درمانی ویتیلیگو شامل درمان‌های دارویی و جراحی است. انتقال ملانوسیت‌های کشت داده نشده، یک روش جراحی مؤثر و جدید در درمان ویتیلیگوی ثابت است. در این مطالعه، تأثیر سوسپانسیون سلولی ملانوسیت در محیط Jokliks در درمان ویتیلیگوی ثابت مقاوم بررسی گردید.روش‌ها: 10 بیمار دچار ویتیلیگوی ثابت مقاوم به درمان استاندارد، تحت درمان انتقال سوسپانسیون سلولی ملانوسیت در محیط Jokliks قرار گرفتند. این افراد، 3 هفته، 3 ماه و 6 ماه پس از عمل ارزیابی شدند و پیگمانتاسیون مجدد نمره‌دهی شد.یافته‌ها: در این مطالعه، 10 بیمار و در مجموع، 16 ضایعه تحت بررسی قرار گرفتند. میزان متوسط پیگمانتاسیون مجدد در بیماران، در هفته سوم 14 درصد، در ماه سوم 37 درصد و در پایان ماه ششم 42 درصد بود که از نظر درجه‌بندی، معادل پیگمانتاسیون مجدد ضعیف (کمتر از 50 درصد) در نظر گرفته می‌شود.نتیجه‌گیری: نتایج این مطالعه‌ی مقدماتی نشان می‌دهد که استفاده از سوسپانسیون سلولی در محیط Jokliks، به ویژه در بیماران با تیپ پوستی روشن، به تنهایی و بدون درمان کمکی (مثل نور درمانی) در بیماران مبتلا به ویتیلیگوی ثابت با پاسخ درمانی کمتر از 50 درصد همراه می‌باشد.

Background: Vitiligo is a cosmetically disfiguring acquired depigmenting disorder caused by the loss of functional melanocytes in the epidermis. Various approaches used for the treatment of vitiligo can be classified as medical and surgical therapies. Non-cultured autologous melanocyte transplantation is a new and effective surgical treatment for stable vitiligo. This study aimed to evaluate the effect of the melanocyte cell suspension in Jokliks medium in the treatment of stable resistant vitiligo.Methods: Ten patients with stable vitiligo, resistant to standard treatment, underwent the treatment via melanocyte cell suspension in Jokliks medium. The repigmentation was assessed during 3 weeks, and 3 and 6 months. Findings: The mean repigmentation of 10 patients with 16 lesions was 14% in 3 weeks, 37% in 3 month and 42% in 6 month that was poor repigmentation (less than 50%).Conclusion: Results of this pilot study showed less than 50% repigmentation using melanocyte cell suspension in Jokliks medium, especially in light skin type patients, without using adjunct treatments like phototherapy.
