بررسی میزان اختلاف نظر بین مشاهده‌گرها در درجه‌بندی و مرحله‌بندی هپاتیت مزمن بر اساس دو سیستم‌ Knodell Ishak (Modified HAI) و Batts-Ludwig

چکیده
مقدمه: ارزیابی قابل اعتماد پاتولوژی بیوپسی کبد در بیماران هپاتیت مزمن، برای تعیین شدت و میزان پیشرفت بیماری و تصمیم‌گیری در مورد درمان اهمیت دارد. این تحقیق، با هدف بررسی میزان تفاوت نظر بین مشاهده‌گرها در طبقه‌بندی هپاتیت مزمن بر اساس 2 مورد از سیستم‌های پرکاربرد انجام شد.روش‌ها: 3 مشاهده‌گر به طور جداگانه بر اساس دو سیستم Knodell Ishak (Modified HAI یا Modified histology activity index) و Batts-Ludwig، درجه (Grade) و‌ مرحله‌ی (Stage) 54 نمونه‌ی بیوپسی کبد از بیماران مبتلا به هپاتیت مزمن را تعیین نمودند. در نهایت، داده‌ها با هم مقایسه گردید.یافته‌ها: بیشترین توافق نظر در مرحله‌بندی بر اساس Knodell Ishak (Modified HAI) (ضریب همبستگی Kendall = 90/0، 001/0 > P) و کمترین توافق نظر در مرحله‌بندی بر اساس Batts-Ludwig (ضریب همبستگی Kendall = 63/0، 001/0 > P) به دست آمد.نتیجه‌گیری: در هر دو سیستم، توافق نظر بهتری بین مشاهده‌گرها در مرحله‌بندی نسبت درجه‌بندی وجود داشت. هر دو سیستم دارای توافق نظر قابل قبولی بین مشاهده‌گرها هستند، اما سیستم Knodell Ishak (Modified HAI) از نظر توافق نظر بین مشاهده‌گرها بر سیستم Batts-Ludwig برتری دارد.

Background: Reliable assessment of liver histopathology in patients with chronic hepatitis is important for assessing the severity of the disease. This study aimed to survey the interobserver variability of liver biopsy assessment performed via two of the most commonly used systems.Methods: 54 liver biopsies from patients with chronic hepatitis were scored by three independent observers using Knodell Ishak [modified histology activity index (HAI)] and Batts-Ludwig systems.Findings: The best agreement was seen for Knodell Ishak’s staging (Kendall’s correlation coefficient (r) = 90%, P < 0.001). The worst agreement was seen for Batts-Ludwig grading. (r = 63%, P < 0.001).Conclusion: We conclude that both systems showed better agreement for staging than for grading; while both systems produced reasonable agreement between the observers, Knodell Ishak system showed greater results.
