بررسی مقایسه‌ای اثر سه داروی دولوکستین، ونلافاکسین و پره‌گابالین بر روی علایم حسی بیماران مبتلا به پلی‌نوروپاتی دیابتی

چکیده

مقدمه: دردهای نوروپاتیک محیطی، یک اختلال شایع در بیماران مبتلا به دیابت است که باعث ناتوانی قابل توجهی در بیماران می‌شود. درمان‌های مختلفی برای درمان این عارضه بیان شده است. در مطالعه‌ی حاضر با هدف یافتن بهترین دارو برای درمان این بیماران (از لحاظ قدرت کاهش درد در کوتاه مدت)، مقایسه‌ای بین داروهای دولوکستین، ونلافاکسین و پره‌گابالین انجام شد.روش‌ها: در این مطالعه، 115 بیماران مبتلا به پلی‌نوروپاتی دیابتی شرکت داشتند. بیماران به صورت تصادفی در سه گروه دریافت کننده‌ی دولوکستین (روزانه یک عدد قرص 60 میلی‌گرمی)، ونلافاکسین (روزانه یک عدد قرص 5/37 میلی‌گرمی) و پره‌گابالین (روزانه یک عدد قرص 300 میلی‌گرمی) تقسیم شدند. در پایان هفته‌ی ششم، شدت درد و مدت درد (ساعت در روز) در سه گروه بررسی شد.یافته‌ها: شدت درد پس از 6 هفته درمان در گروه دولوکستین به صورت معنی‌داری کمتر از گروه‌های دیگر بود (030/0 = P). اختلاف میانگین شدت درد در بیماران سه گروه کاهش داشت، اما معنی‌دار نبود (174/0 = P). در خصوص مدت زمان درد نیز مشخص شد که بعد از درمان، مدت زمان درد میان گروه‌های مختلف اختلاف معنی‌داری نداشت (050/0 < P).نتیجه‌گیری: هیچ تفاوتی در نتایج درمان با استفاده از داروهای دولوکستین، ونلافاکسین و پره‌گابالین در کاهش درد در بیماران مبتلا به پلی‌نوروپاتی دیابتی دیده نشد. از این رو، می‌توان بر اساس شرایط بیمار، یکی از داروهای مورد نظر را تجویز کرد.

A Comparative Study on the Effect of Duloxetine Hydrochloride, Venlafaxine Hydrochloride, and Pregabalin on the Sensory Symptoms in Patients with Diabetic Polyneuropathy

Background: Peripheral neuropathic pain is a common disorder in patients with diabetes mellitus, which causes significant disability. Various treatments for this disorder have been proposed. This study aimed to find the best medicine for treating these patients (in short-term pain relief) via a comparison between duloxetine, venlafaxine, and pregabalin.Methods: 115 patients with diabetic polyneuropathy participated in this study. Patients were randomly divided into three groups of duloxetine (60 mg daily), venlafaxine (37.5 mg daily), and pregabalin (300 mg daily). At the end of the sixth weeks, pain severity and duration (hours per day) were evaluated and compared in three groups.Findings: The severity of pain after 6 weeks of treatment in the duloxetine group was significantly lower than the other groups (P = 0.030). However, the mean difference in pain intensity in the three groups was reduced, but there was no statistically significant difference (P = 0.174). Regarding the duration of pain, it was found that after the treatment, the duration of pain was not significantly different between the groups (P < 0.050).Conclusion: There was no difference in the results of treatment with duloxetine, venlafaxine, and pregabalin in reducing pain in patients with diabetic polyneuropathy. Therefore, one of the drugs can be prescribed according to the conditions of the patient.
