اثربخشی آموزش روانی بر کاهش شدت علایم شیدایی و افزایش بینش بیماران مبتلا به اختلال دو قطبی نوع 1 در مرحله‌ی شیدایی

چکیده

مقدمه: هدف از انجام مطالعه‌ی حاضر، بررسی اثربخشی آموزش روانی بر افزایش بینش و کاهش علایم شیدایی بیماران مبتلا به اختلال دو قطبی در مرحله‌ی شیدایی بود.روش‌ها: پژوهش حاضر از نوع نیمه‌ آزمایشی به صورت پیش‌آزمون- پس‌آزمون با گروه شاهد بود. جامعه‌ی آماری این مطالعه، شامل بیماران دو قطبی بستری در مرکز آموزشی- درمانی شفا در شهر رشت بود که از فروردین 1395 تا شهریور 1396 در این مرکز بستری بودند. نخست، آزمودنی‌ها با استفاده از معیارهای ورود و خروج غربال شدند. سپس، 24 بیمار (شامل 12 بیمار در هر گروه) بر پایه‌ی نمونه‌گیری در دسترس انتخاب شدند و به طور تصادفی در دو گروه مورد و شاهد قرار گرفتند. گروه مورد، علاوه بر دارودرمانی، آموزش روانی را به مدت 8 جلسه‌ی هفتگی دریافت کردند و گروه شاهد تنها دارودرمانی دریافت کردند. علایم شیدایی و بینش بیماران قبل و بعد از مداخله با استفاده از مقیاس سنجش مانیای Young (Young mania rating scale یا YMRS) و مقیاس سنجش آگاهی نداشتن از اختلال روانی (Scale to assess unawareness in mental disorder یا SUMD) ارزیابی شد. داده‌های پژوهش با استفاده از آزمون MANOVA تحلیل شد.یافته‌ها: گروه مورد به لحاظ آماری افزایش معنی‌داری را در میزان بینش نشان داد، اما هیچ تفاوت معنی‌داری بین علایم شیدایی گروه مورد و شاهد دیده نشد.نتیجه‌گیری: آموزش روانی در کنار دارودرمانی در افزایش بینش مؤثر است، اما هیچ تأثیری بر روی علایم شیدایی بیماران مبتلا به اختلال دو قطبی نمی‌گذارد

Background: The purpose of this study was to evaluate the effectiveness of psychoeducation on increasing insight and reducing mania symptoms in patients with bipolar I disorder in mania phase.Methods: This was a quasi-experimental study with pretest-posttest design and control group. The statistical population included patients with bipolar I disorder in Shafa hospital in Rasht City, Iran, from April 2016 to September 2017. First, the subjects were screened using inclusion and exclusion criteria; then, 24 patients were selected based on convenience sampling and randomly assigned to equal intervention and control groups. In addition to medication, the intervention group received psychoeducation for 8 sessions per week, and the control group received only medication. The present study was a semi-experimental design with pre-test, post-test and control group. Mania and introspection symptoms were assessed before and after intervention using Young's Mania Rating Scale (YMRS) and Scale to Assess Unawareness in Mental Disorder (SUMD). The data were analyzed using multivariate analysis of variance.Findings: The intervention group showed statistically significant increase in insight, but no significant difference was found between the mania symptoms among the intervention and control groups.Conclusion: Psychoeducation as well as pharmacotherapy was effective in increasing insight, but had no effect on the mania symptoms among the patients with bipolar I disorder.
