چکیده
مقدمه: در این پژوهش، الگوی مقاومت آنتیبیوتیکی سویههای استافیلوکوکوس اورئوس مقاوم به متیسیلین (Methicillin-resistant Staphylococcus aureus یا MRSA) جدا شده از نمونههای بالینی مورد بررسی گرفت.روشها: این مطالعه بر روی نمونههای بالینی یک بیمارستان آموزشی در شهر اصفهان انجام گردید. سویههای استافیلوکوکوس اورئوس با استفاده از روشهای استاندارد آزمایشگاهی و مولکولی شناسایی شدند. بررسی الگوی مقاومت آنتیبیوتیکی سویههای MRSA با استفاده از روش انتشار دیسک بر اساس پروتکل انستیتو استانداردهای آزمایشگاهی و بالینی (Clinical and Laboratory Standards Institute یا CLSI) انجام گرفت. از روش Polymerase chain reaction (PCR) به منظور شناسایی ژن mecA استفاده گردید.یافتهها: 125 سویهی استافیلوکوکوس اورئوس با روشهای بیوشیمیایی و مولکولی شناسایی گردید. فراوانی سویههای MRSA، 40 درصد و بیشترین جدایههای MRSA مربوط به نمونههای زخم بود. بیشترین مقاومت سویههای MRSA پس از مقاومت به پنیسیلین (100 درصد)، به آنتیبیوتیکهای ارتیرومایسین و سیپروفلوکساسین (92 درصد) اختصاص داشت. 17 الگوی متفاوت آنتیبیوتیکی در میان سویههای MRSA شناسایی شد.نتیجهگیری: 40 درصد سویههای استافیلوکوکوس اورئوس جدا شده در تحقیق حاضر، مقاوم به متیسیلین بودند و نمونههای جداسازی شده از زخم، بالاترین میزان جداسازی را داشتند.

Background: The aim of this study was to investigate the prevalence of antibiotic resistance in methicillin-resistant Staphylococcus aureus (MRSA) strains isolated from clinical specimens.Methods: This study was performed on clinical samples in a teaching hospital in Isfahan City, Iran. Isolates of Staphylococcus aureus were identified using standard laboratory and molecular assessments. Antibiotic resistance pattern of MRSA isolates was evaluated by disk diffusion method based on the Clinical and Laboratory Standards Institute (CLSI) protocol. Polymerase chain reaction (PCR) technique was performed for the identification of mecA gene.Findings: 125 strains of Staphylococcus aureus were identified by biochemical and molecular methods. The prevalence of methicillin-resistant strains was 40%. The highest number of MRSA isolates was related to wound samples, and the highest resistance of MRSA strains after resistance to penicillin (100%), was to the antibiotics erythromycin and ciprofloxacin (92%). 17 different antibiotic patterns were identified among MRSA strains.Conclusion: The data revealed that 40% of Staphylococcus aureus strains were methicillin-resistant. The highest number of MRSA isolates was related to wound samples.
