اثرات داروی متفورمین در بیماران دیابتی مبتلا به عفونت همزمان کووید-19: یک مطالعه‌ی مروری

مقدمه: در بیماران مبتلا به COVID-19، دیابت یکی از شایع‌ترین بیماری‌هایی است که با پیامدهای بدی همراه می‌باشد. یافته‌ها نشان می‌دهد که مرگ و میر در افراد مبتلا به دیابت که متفورمین مصرف می‌کنند در مقایسه با کسانی که متفورمین مصرف نمی‌کنند، کاهش یافته است. این مطالعه با هدف ارائه‌ی شواهد معتبر و به‌روز در مورد اثرات مصرف داروی متفورمین در بیماران مبتلا به دیابت به عفونت همزمان با کووید-19، انجام گرفت.
روش‌ها: در این بررسی از پایگاه‌های PubMed، Embase،  Cochrane و Google Scholar از دسامبر 2019 جهت جستجوی مطالعات استفاده کردیم. با استفاده از کلمات کلیدی «متفورمین» یا «بیگوانیدها» و «بیماری کرونا ویروس 2019» یا "COVID-19" به جستجوی سیستماتیکی از متون با زبان انگلیسی اقدام شد.
یافته‌ها: طبق مطالعات بررسی شده، در میان افراد دیابتی مبتلا به عفونت همزمان با COVID-19، اکثراً کاهش مرگ و میر را در مصرف‌کنندگان متفورمین در مقایسه با افراد غیر مصرف‌کننده  گزارش نموده‌اند. همچنین نتایج نشان می‌دهد که متفورمین با افزایش حساسیت به انسولین می‌تواند اثر مهاری بر روی ویروس‌ها داشته باشد.
نتیجه‌گیری: نتایج این مطالعه نشان داد که مصرف متفورمین در بیماران دیابتی مبتلا به کووید-19 با مرگ و میر کمتری همراه بود. البته کارآزمایی‌های تصادفی‌سازی و کنترل‌شده برای تأیید این یافته‌ها مورد نیاز است.

The Effects of Metformin Drug in Diabetic Patients Suffering from Concomitant Infection with Covid-19; A Review Study

Background: Diabetes is one of the most common co-morbidities and is associated with worse results in patients with coronavirus disease 2019 (COVID-19). The results showed that mortality was reduced among diabetics taking metformin compared with those not taking metformin. Therefore, the present study was conducted with the aim of providing reliable and up-to-date evidence about the effects of metformin drug in diabetic patients suffering from concomitant infection with COVID-19.
Methods: In this review, we searched PubMed, Embase, Cochrane and Google Scholar databases starting in December 2019 to find studies. We carried out a systematic search of English texts using the keywords "metformin" or "biguanides" and "coronavirus disease 2019" or "COVID-19". After reviewing the titles, summaries and complete texts, 28 studies were eventually included in the study.
Findings: The review found that most studies showed reduced mortality among metformin users compared to non-users among diabetics with COVID-19. In addition, the results indicate that metformin may inhibit viruses by increasing insulin susceptibility. However, some studies do not support the impact of metformin on
mortality reduction.
Conclusion: Results from this study show that metformin use in diabetic patients with COVID-19 is associated with decreased mortality. Of course, randomized controlled trials are necessary to confirm this outcome.
