تأثیر مشاوره مادر محور بر خودکارآمدی و نگرانی دوران بارداری : مطالعه نیمه تجربی توانمندسازی مبتنی بر نیاز در مادران نوجوان شهر زاهدان

مقدمه: افزایش نگرانی‌ها و خودکارآمدی پایین با افزایش پیامدهای نامطلوب بارداری در نوجوانان در ارتباط است. مطالعه‌ی حاضر با هدف تعیین تأثیر مشاوره‌ی مادرمحور برخودکار آمدی زایمان و نگرانی دوران بارداری در مادران نوجوان انجام گردید.
روش‌ها: این مطالعه‌ی نیمه‌تجربی در سال 1402 بر روی 70 نوجوان پرایمی‌گراوید مراجعه‌کننده به مراکز جامع سلامت زاهدان انجام شد. مداخله شامل6 جلسه مشاوره گروهی به مدت 60-45 دقیقه در سه ماهه‌ی دوم بارداری بود. ابزار گردآوری داده‌ها پرسشنامه‌ی خودکارآمدی زایمان و نگرانی دوران بارداری کمبریج بود. تجزیه و تحلیل بر روی داده‌های 32 نفر در گروه مداخله و 30 نفر در گروه شاهد با استفاده Independent Samples T-test و Paired T-test با سطح معنی‌داری 05/0 > P استفاده گردید.
یافته‌ها: قبل از مداخله، میانگین نمره‌ی خودکارآمدی در گروه مداخله (16/75 ± 93/03) و شاهد (15/24 ± 99/1) اختلاف معنی‌داری نداشت (0/14 = P) اما پس از انجام مداخله، اختلاف معنی‌داری بین گروه مداخله (12/74 ± 131/53) و شاهد (14/86  ±100/4) مشاهده شد (0/001 >P). از نظر میانگین نمره‌ی نگرانی قبل از مداخله، بین گروه مداخله (10/9 ± 81/18) و شاهد (7/23 ±84/5) اختلاف معنی‌داری وجود نداشت (0/16 = P) اما پس از انجام مداخله، اختلاف معنی‌داری بین گروه مداخله (9/86 ± 54/09) و شاهد (6/41 ± 86/3) وجود داشت (0/001 > P).
نتیجه‌گیری: مشاوره‌ی مادر‌محور، باعث افزایش خودکارآمدی زایمان و کاهش نگرانی‌های دوران بارداری در نوجوانان باردار می‌شود. برگزاری جلسات مشاوره مبتنی بر نیاز مادران در کنار مراقبت‌های معمول برای بهبود مراقبت‌ها توصیه می‌شود.

Background: Increased worries and low self-efficacy are associated with increased adverse pregnancy outcomes in adolescents. The present study was conducted to determine the effect of mother-centered counseling on the outcome of childbirth and anxiety during pregnancy in teenage mothers.
Methods: This quasi-experimental study was conducted in 2023 on 70 primigravida pregnant teenagers referred to comprehensive health centers in Zahedan. The intervention consisted of 6 group counseling sessions for 45-60 minutes in the second trimester of pregnancy. The tools for data collection were the Cambridge Worry Scale and Childbirth Self-Efficacy Inventory questionnaires. Analysis was done on the data of 32 people in the intervention group and 30 people in the control group. Independent T-tests and Paired T-tests with a significance level of P < 0.05 were used for analysis.
Findings: Before the intervention, the mean score of self-efficacy in the intervention group (93.03 ± 16.75) and control group (99.15 ± 15.24) had no significant difference (P = 0.14), but after the intervention, a significant difference was observed between the intervention group (131.53 ± 12.74) and control group (100.4 ± 14.86) (P < 0.001). There was no significant difference between the intervention group (81.18 ± 10.9) and the control group (84.5 ± 7.23) in terms of mean worry score before intervention (P = 0.16). However, after the intervention, there was a significant difference between the intervention group (54.09 ± 9.86) and the control group (86.3 ± 6.41) (P < 0.001).
Conclusion: Mother-centered counseling increases the self-efficacy of childbirth and reduces worries during pregnancy in pregnant teenagers. It is recommended to hold counseling sessions based on mothers' needs in addition to routine care to improve care.
