بررسی تغییرات قبل و بعد از همودیالیز بر روی رتین بیماران نارسایی مزمن کلیه با روش آنژیوگرافی توموگرافی انسجام نوری

چکیده
مقدمه: همودیالیز، می‌تواند باعث تغییر در همودینامیک مایعات بدن و تأثیر بر روی چشم گردد. در این مطالعه برای بررسی تأثیر همودیالیز بر شاخص‌های رتین چشم از روش آنژیوگرافی توموگرافی انسجام نوری  قبل و بعد از همودیالیز اجرا گردید.
روش‌ها: این مطالعه به صورت مقطعی بر روی 27 بیمار مبتلا به نارسایی مزمن کلیوی در بخش دیالیز بیمارستان هاجر شهرکرد اجرا گردید. معیارهای ورود شامل سابقه‌ی همودیالیز به مدت حداقل 1 سال و عدم ابتلا به بیماری قبلی چشم از قبیل گلوکوم، رتینوپاتی، بیماری‌های شبکیه یا جراحی قبلی بر روی چشم و معیار خروج ابتلا به دیابت بود. برای جمع‌آوری اطلاعات از چک‌لیست استفاده گردید. بررسی شاخص‌های رتین با استفاده از دستگاه آنژیوگرافی توموگرافی انسجام نوری قبل و بعد دیالیز انجام گرفت.
یافته‌ها: شاخص‌های ضخامت رتین و تراکم عروق سطحی رتین قبل و بعد از همودیالیز تغیر معنی‌داری نداشتند (05/0 < P). نتایج تراکم عروق عمقی رتین نشان داد میانگین پارافووا نیمه تحتانی از 61/49µmقبل از همودیالیز به 44/22  µmبعد از همودیالیز (019/0 = P). میانگین پارا فووا تمپورال از 81/18  µmبه (0/05 = P)، 42/95  µmمیانگین پارا فووا نازال از 48/23  µmبه (0/022 = P)، 86/42 µm و میانگین پری فووا نازال از 43/51 µm به 45/01  µmکاهش یافت.
نتیجه‌گیری: با توجه به اینکه OCTA یک تکنیک تصویربرداری جدید است و نتایج مطالعات در این زمینه محدود است، نتایج باید با احتیاط تفسیر شود. بر اساس یافته های حاصل از این پژوهش همودیالیز تغیرات چندان قابل توجهی بر شاخص های رتین بیماران ایجاد نکرده است.

چکیده English

Background: Hemodialysis can change the hemodynamics of body fluids and its effect on the eye retina. In this study, coherence tomography angiography was performed to evaluate the effect of hemodialysis on retinal parameters of the eye before and after hemodialysis.
Methods: This cross-sectional study was performed on 27 patients with chronic renal failure in the dialysis ward in Shahrekord. Inclusion criteria included a history of hemodialysis for at least 1 year and no previous eye disease such as glaucoma, retinopathy, retinal disease or previous eye surgery and exclusion criteria for diabetes. A checklist was used to collect data. Retinal indices were assessed using optical coherence tomography angiography before and after dialysis. Data were analyzed using SPSS24 software.
Findings: The indices of retinal thickness and retinal superficial vascular density did not change significantly before and after hemodialysis (P> 0.05). The results of deep retinal vascular density showed that the mean Parafovea Inferior hemisphere change from 49.61 before hemodialysis to 44.22 µm after hemodialysis (P = 0.019). The mean of Parafovea temporal from 81.18 µm to 42.95 µm (P = 0.05), the mean of Parafovea nasal from 48.23 to 42.86 (P=0.022) and the average Perifovea nasal from 51.43 بهm 01m decreased 01/45 (P = 021).
Conclusion: Given that OCTA is a new imaging technique and the results of studies in this field are limited, the results should be interpreted with caution. Based on the findings of this study, it was concluded that hemodialysis did not significantly change the retinal parameters of patients.
