سازگار‌سازی استانداردهای اعتباربخشی ملی با ساختار عملکردی بیمارستان‌های ایران برای بیمارستان‌های کوچک: یک مطالعه ترکیبی

چکیده
مقدمه: در ایران و در سالیان اخیر، ملاک اعتباربخشی بیمارستان ها، نسل پنجم راهنمای جامع استانداردهای اعتبار بخشی ملی بوده است که برای همه انواع بیمارستان‌ها از نظر اندازه به طور یکسان استفاده می‌گردد. هدف این پژوهش، سازگارسازی و انطباق راهنمای مذکور با ساختار عملکردی بیمارستان‌های کوچک بود.
روش‌ها: این پژوهش، یک مطالعه‌ی ترکیبی از نوع متوالی اکتشافی بود که در دو فاز انجام شد. فاز اول (فاز کیفی) مصاحبه‌ی عمیق نیمه ساختار یافته با 22 نفر مشارکت کننده شامل مدیران داخلی و مدیران بهبود کیفیت بیمارستان‌های هدف، ارزیابان کشوری اعتباربخشی و کارشناسان مرکز نظارت و اعتباربخشی امور درمان وزارت بهداشت بود که با روش نمونه‌گیری هدفمند با حداکثر تنوع انتخاب شدند. در فاز دوم (فاز کمی) برای اعتبارسنجی یافته‌های به دست آمده، از پنل خبرگان و تکنیک دلفی تعدیل شده استفاده شد.
یافته‌ها: راهنمای جامع اعتباربخشی ملی بیمارستان‌های ایران (ویرایش پنجم)، 505 سنجه دارد که مطابق یافته‌های این پژوهش، 82 مورد آن (معادل 2/16 درصد)، برای بیمارستان کوچک قابل حذف یا قابل ویرایش‌اند. از این تعداد، 71 سنجه قابلیت حذف و 11 سنجه، قابلیت ویرایش دارند. دلایل اصلی حذف/ ویرایش سنجه‌ها، محدودیت‌های فیزیکی و ساختمانی این بیمارستان‌ها، فقدان کادر درمان به خصوص پزشکان متخصص و همچنین نداشتن تجهیزات پزشکی بود.
نتیجه‌گیری: با در نظر گرفتن ویژگی‌های خاص بیمارستان‌های کوچک، می‌توان نسل پنجم راهنمای جامع استانداردهای اعتباربخشی را بازنویسی کرد. این اقدام می‌تواند فرایند اعتباربخشی را تسهیل کرده و هزینه‌های اضافی را کاهش دهد.

Customization of the Comprehensive Guide to National Accreditation Standards for Iranian Hospitals for Small Hospitals: A Mixed Methods Study

چکیده English

Background: In Iran, in recent years, the criterion for hospital accreditation has been the fifth generation of the Comprehensive Guide to National Accreditation Standards, which is used equally for all types of hospitals regardless of size. The aim of this research was to customize and adapt the aforementioned guide to the unique conditions and characteristics of small hospitals.
Methods: This research was a mixed quantitative-qualitative study conducted in two phases. The first phase (qualitative phase) was a semi-structured in-depth interview with 22 participants, including internal managers and quality improvement managers of the target hospitals, national accreditation assessors, and experts from the Center for Monitoring and Accreditation of Medical Affairs of the Ministry of Health, who were selected using a purposive sampling method with maximum diversity. In the second phase (quantitative phase), a modified Delphi technique was used to validate the findings.
Findings: The Comprehensive Guide to National Accreditation of Iranian Hospitals (5th Edition) has 505 issues, of which, according to the findings of this study, 82 (equivalent to 16.2%) can be deleted/edited for small hospitals. Of these, 71 issues can be deleted and 11 issues can be edited. The main reasons for deleting/editing issues were the physical and structural limitations of these hospitals, the lack of medical staff, especially specialist doctors, and the lack of medical equipment.
Conclusion: Taking into account the specific characteristics of small hospitals, the fifth generation of the Comprehensive Accreditation Standards Guide can be rewritten. This can simplify the accreditation process and reduce additional costs.
