مواجهه‌ی بالینی زودهنگام در آموزش دندانپزشکی: یک مرور دامنه‌ای نظام‌مند

چکیده
مقدمه: مواجهه‌ی بالینی زودهنگام (Early Clinical Exposure) ECE به‌طور فزاینده‌ای به‌عنوان یک راهبرد آموزشی مؤثر برای پر کردن شکاف میان یادگیری پیش‌بالینی (Preclinical Learning) و عملکرد بالینی (Clinical Practice) در آموزش دندانپزشکی شناخته می‌شود.
روش‌ها: این مرور نظام‌مند با هدف ترکیب و جمع‌بندی شواهد جهانی درباره‌ی تأثیر مواجهه بالینی زودهنگام بر دانشجویان مقطع کارشناسی دندانپزشکی انجام شد. جست‌وجوی نظام‌مند در پایگاه‌های PubMed، Scopus و Web of Science از زمان آغاز به کار این پایگاه‌ها تا اکتبر ۲۰۲۵ انجام شد. مطالعاتی وارد مرور شدند که مداخلات مواجهه بالینی زودهنگام را در میان دانشجویان دندانپزشکی، در هر مرحله‌ای از آموزش پیش‌بالینی، ارزیابی کرده و پیامدهای آموزشی یا حرفه‌ای را گزارش کرده بودند. داده‌ها استخراج و به‌صورت روایتی (Narrative Synthesis) ترکیب شدند و کیفیت مطالعات با استفاده از ابزارهای مؤسسه جوانا بریگز (Joanna Briggs Institute) JBI ارزیابی شد..
یافته‌ها: 12 مطالعه معیارهای ورود را داشتند که شامل کارآزمایی‌های تصادفی‌سازی‌شده کنترل‌شده، مطالعات شبه‌آزمایشی، پیمایش‌های مقطعی و پژوهش‌های کیفی بودند. مداخلات مواجهه بالینی زودهنگام از تماس مستقیم با بیمار و چرخش‌های بالینی تا ماژول‌های ساختارمند پیش‌بالینی، یادگیری با کمک همتایان و نمایش‌های مبتنی بر ویدئو متغیر بودند. بیشتر مطالعات، بهبود معناداری را در دانش، مهارت‌های روانی‌- حرکتی، اعتمادبه‌نفس، توانایی‌های ارتباطی و انگیزش دانشجویان گزارش کردند. دانشجویان همچنین آمادگی بیشتر برای ورود به عملکرد بالینی و هویت حرفه‌ای قوی‌تری را گزارش کردند. چالش‌های رایج شامل کمبود اعضای هیأت علمی، دشواری‌های ادغام در برنامه درسی و استفاده محدود از ابزارهای ارزیابی اعتبارسنجی‌شده بود.
نتیجه‌گیری: مواجهه‌ی بالینی زودهنگام تأثیری به‌طور مداوم مثبت بر حوزه‌های مختلف آموزش دندانپزشکی دارد و گذار روان‌تر از یادگیری کلاسی به عملکرد بالینی را تسهیل می‌کند. گنجاندن مؤلفه‌های ساختارمند و بیمارمحور مواجهه بالینی زودهنگام در برنامه‌های درسی دندانپزشکی برای بهبود پیامدهای آموزشی و توسعه حرفه‌ای قویاً توصیه می‌شود.
