مقایسه‌ی حساسیت اخلاقی دستیاران رشته‌های جراحی و غیرجراحی: یک مطالعه‌ی مقطعی در بیمارستان‌های آموزشی اصفهان

چکیده
مقدمه: حساسیت اخلاقی، از ضرورت‌های پایه‌ای خدمت‌رسانی حرفه‌ای در حوزه‌ی سلامت است که به پزشکان در شناسایی تعارض‌های اخلاقی و اتخاذ تصمیم‌های بالینی درست کمک می‌کند. مطالعه‌ی حاضر با هدف تعیین و مقایسه‌ی میانگین نمره‌ی حساسیت اخلاقی بین دستیاران رشته‌های جراحی و غیرجراحی در بیمارستان‌های آموزشی اصفهان انجام شد.
روش‌ها: مطالعه از نوع مقطعی در سال ۱۴۰۲ در بیمارستان‌های الزهرا(س) و کاشانی اصفهان اجرا شد. ۱۰۰ دستیار شاغل در رشته‌های مختلف (۲۶ نفر جراحی و ۷۴ نفر غیرجراحی) به روش نمونه‌گیری خوشه‌ای تصادفی انتخاب شدند. ابزار گردآوری داده‌ها، پرسشنامه استاندارد حساسیت اخلاقی لوتزن (MSQ) شامل ۲۵ گویه در ۶ بعد بود. داده‌ها با استفاده از آزمون‌های تحلیل چند متغیری تحلیل شد.
یافته‌ها: میانگین و انحراف معیار نمره‌ی کل حساسیت اخلاقی دستیاران 12/83±67/02 گزارش شد که نشان‌دهنده‌ی سطح متوسط است. یافتهها نشان داد حساسیت اخلاقی در ۱۵ درصد دستیاران پایین، ۶۷ درصد متوسط و ۱۸ درصد در سطح بالا قرار دارد. بین نمره‌ی کل حساسیت اخلاقی و زیرمقیاسهای آن در دستیاران رشته‌های جراحی و غیرجراحی تفاوت آماری معناداری مشاهده نشد (0/05 < P). همچنین متغیرهایی چون جنسیت و سال تحصیلی، بر میزان حساسیت اخلاقی شرکت‌کنندگان تأثیر معناداری نداشتند (0/05 < P).
نتیجه‌گیری: حساسیت اخلاقی بیشتر دستیاران در رشته‌های تخصصی مورد مطالعه در حد متوسط ارزیابی شد. با توجه به اهمیت این مهارت در کیفیت مراقبت از بیمار و تصمیم‌گیری‌های اخلاقی، بازنگری در برنامه‌های آموزشی اخلاق پزشکی و اجرای دوره‌های عملی و مورد محور جهت ارتقای حساسیت اخلاقی پزشکان در دوره دستیاری توصیه می‌شود.

Comparison of Moral Sensitivity between Surgical and Non-Surgical Residents: A Cross-Sectional Study in Teaching Hospitals of Isfahan

Introduction: This study aimed to determine and compare the mean moral sensitivity scores between surgical and non-surgical residents in teaching hospitals in Isfahan.
Methods: This cross-sectional study was conducted in 2023 at Al-Zahra and Kashani Hospitals in Isfahan. A total of 100 residents practicing in various fields (26 surgical and 74 non-surgical) were selected using cluster random sampling. Data collection was performed using the standard Lützén Moral Sensitivity Questionnaire (MSQ), comprising 25 items across six dimensions. Data were analyzed using multivariate analysis tests in SPSS version 16.
Findings: The mean and standard deviation of the total moral sensitivity score of the residents was reported as 67.02 ± 12.83, indicating a moderate level. Findings revealed that moral sensitivity was low in 15%, moderate in 67%, and high in 18% of the residents. No statistically significant difference was observed between the total moral sensitivity score and its subscales between surgical and non-surgical residents (P > 0.05). Additionally, variables such as gender and academic year had no significant effect on the participants' level of moral sensitivity (P > 0.05).
Conclusion: The moral sensitivity of most residents in the studied specialties was evaluated at a moderate level. Given the importance of this skill in the quality of patient care and ethical decision-making, it is recommended to revise medical ethics educational programs and implement practical, case-based courses to enhance the moral sensitivity of physicians during their residency.
