بررسی اثر پانسمان نوین در بهبود زخم وکاهش میزان آمپوتاسیون پای دیابتی

چکیده
مقدمه: زخم پا یکی از مهمترین عوارض دیابت است. برهمین اساس مطالعه حاضر با هدف تعیین تاثیر پانسمان نوین در کاهش میزان آمپوتاسیون پای دیابتی انجام شد.
روش ها: این مطالعه از نوع کوهورت گذشته‌نگر بود. تعداد نمونه برابر با 100 نفر از بیماران بود که بصورت تصادفی ساده انتخاب و وارد مطالعه شدند، سپس بیماران بر اساس نوع مداخله در دو گروه پانسمان سنتی و پانسمان نوین هرکدام 50 نفر بصورت تصادفی تقسیم شدند. اطلاعات بوسیله چک‌لیست و توسط پژوهشگر جمع‌آوری گردید. داده‌ها توسط نرم‌افزار 16SPSS و در سطح معناداری 05/0 مورد بررسی قرار گرفت.
یافته‌ها: میانگین سنی در گروه پانسمان سنتی برابر با 63/8±62/53 سال و در گروه پانسمان نوین برابر با 39/9±94/52 سال بود. 34 نفر از بیماران مرد و 66 نفر زن بودند. میانگین شاخص توده بدنی در گروه پانسمان سنتی برابر با 84/8±86/29 و در گروه پانسمان نوین برابر با 37/9±67/33 بود. آمپوتاسیون در بیماران دو گروه (پانسمان سنتی و نوین) دارای تفاوت معنادار بود و کاهش معنی داری در میزان آمپوتاسیون در گروه پانسمان نوین مشاهده شد (P<0.05). همچنین بین آمپوتاسیون در دو گروه مورد بررسی براساس رتینوپاتی و نفروپاتی تفاوت معنادار وجود داشت (P<0.05).
نتیجه‌گیری: انتخاب پانسمان زخم ممکن است بر عواملی مانند BMI، شدت زخم، عوارض دیابتی و خطر قطع عضو در بین بیماران دیابتی تأثیر بگذارد. این بینش‌ها می‌تواند به ارائه دهندگان مراقبت‌های بهداشتی در تصمیم‌گیری آگاهانه در مورد انتخاب پانسمان زخم برای بیماران دیابتی برای بهینه‌سازی بهبود زخم و بهبود پیامدهای کلی سلامت کمک کند.

چکیده English

Background: Foot ulcer is one of the most important complications of diabetes. Therefore, the present study was performed to determine the effect of new dressings on reducing the rate of amputation of diabetic foot.
Methods: This study was a retrospective cohort design involving a sample of 100 randomly selected patients. The patients were then randomly divided into two groups, each consisting of 50 participants, based on the type of dressing they received: traditional or modern. Data was collected using a checklist and by the researcher. The data were analyzed using SPSS version 16, with a significance level of 0.05.
Findings: The mean age of traditional dressing group was 53.62±8.63 years and in patients new dressings group was 52.94±9.39 years. 34 patients were male and 66 were female. The mean body mass index in the group of traditional dressings was equal to 29.86±8.84 and in the group of new dressings was equal to 33.67±9.37. Amputation was significantly different in patients of the two groups (traditional and new dressings) and a significant reduction in the amputation rate was observed in the new dressings group (P<0.05). There was also a significant difference between amputations in the two groups based on retinopathy and nephropathy (P<0.05).
Conclusion: Wound dressing selection may be influenced by factors such as BMI, wound severity, diabetic complications, and amputation risk among diabetic patients. These insights can help healthcare providers make informed decisions about wound dressing selection for diabetic patients, optimizing wound healing and improving overall health outcomes.
