English

Introduction: Type 1 diabetes mellitus is an autoimmune disorder characterized by pancreatic β-cell destruction and insulin deficiency. Sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, enhances endogenous incretin activity and is widely used in type 2 diabetes. This study evaluated the effects of sitagliptin on glycemic control, insulin requirements, and inflammatory markers in T1DM patients.
Methods: A 12-week, randomized, triple-blind, placebo-controlled trial was conducted at Isfahan University of Medical Sciences. Thirty-two T1DM patients (aged 16–60 years, disease duration <5 years) were randomized to receive sitagliptin 100 mg/day (n=16) or placebo (n=16). Outcomes included weekly blood glucose levels, insulin dose, HbA1c, and inflammatory cytokines (IFN-γ, IL-4). Data were analyzed using paired t-tests, ANCOVA, using SPSS v25.
Results: The sitagliptin group showed a significant reduction in weekly mean blood glucose (−39.6 mg/dL, p<0.001) and IFN-γ levels (p=0.001) compared to placebo. No significant changes were observed in fasting glucose, HbA1c, insulin dose, or IL-4 levels. Adverse events were comparable between groups.
Conclusion: Sitagliptin improved daily glycemic variability and reduced IFN-γ, suggesting anti-inflammatory effects in T1DM. However, it did not alter HbA1c or insulin requirements. Longer-term studies are needed to assess clinical outcomes.
