چکیده
مقدمه: تشخیص دقیق و درمان به موقع عوارض جراحی پس از پیوند کلیه از کارهای مهم دست اندرکاران درمان بیماران پیوند کلیه میباشد. در این مطالعه، لیک ادراری به عنوان یکی از عوارض شایع و مهم پیوند کلیه مورد بررسی قرار گرفت تا شاید بتوان میزان بروز آن را کاهش داد. روشها: این مطالعه بر روی 248 بیماری که از اولین پیوند کلیه در بیمارستان الزهرا (س) اصفهان (تیر ماه 1382) تا اسفند ماه 1385 تحت این عمل جراحی قرار گرفتهاند انجام شد. روش نمونهگیری به صورت سرشماری و با استفاده از پرسشنامه بود و دادههای آن به وسیلهی نرمافزار SPSS مورد تجزیه و تحلیل قرار گرفت. یافتهها: از مجموع 248 بیمار مورد مطالعه، 12 مورد (8/4درصد) به لیک ادراری، 7 مورد (8/2 درصد) به انسداد ادراری و 7 مورد (8/2 درصد) به عفونت زخم دچار شده بودند. ارتباط آماری معنیداری بین میزان بروز لیک ادراری، انسداد و لیک ادراری و عفونت زخم وجود داشت ولی ارتباط بین میزان بروز لیک ادراری و بیماریهای زمینهای فرد گیرنده (دیابت، پرفشاری خون و بیماری ایسکمیک قلبی)، جنس و سن فرد دهنده و یا کاداور، کاداور یا دهندهی زنده، خویشاوند و غیر خویشاوند بودن فرد دهنده، میزان دوز کورتون مصرفی و مدت زمان بیهوشی معنیدار نبود. نتیجهگیری: بین میزان بروز لیک ادراری با میزان انسداد ادراری و میزان بروز عفونت زخم ارتباط معنیداری وجود دارد. به نظر میرسد که تکنیک جراحی (نه سایر فاکتورهای همراه) علت اصلی تعیینکنندهی میزان بروز لیک ادراری پس از عمل جراحی پیوند کلیه میباشد. واژگان کلیدی: فیستول ادراری، انسداد ادراری، عفونت زخم جراحی، عوامل تکنیکی

چکیده English
Early and accurate diagnosis and management of surgical complications occurring after kidney transplantation are mandatory for the team managing these patients. Urinary leakage is one of the most important complications. In this paper we determine the incidence of urine leakage and its related factors in order to find ways for reducing this complication. Background: This descriptive study was performed in Al-zahra hospital, affiliated to Isfahan University of Medical Sciences. Data was collected by reviewing files of all the patients transplanted in this center from 2003 to 2008 and they were recorded in questionnaires. The data was analyzed by SPSS software. Methods: Of the total of 248 patients, 12 (4.8%) had urine leakage, 7 (2.8%) had urine obstruction and 7 (2.8%) had wound infection. There was a statistically significant relation between leakage and obstruction of the urinary system, as well as between urine leakage and wound infection. There was no statistically significant relation between the incidence of urine leakage and the recipient's age, sex and underlying diseases; age and sex of the donor; cadaver/alive or relative/non-relative donors, the dosage of corticosteroids, and the duration of anesthesia. Findings: It is suggested that the technique of the surgery, not the other associated factors of the donor and recipient, is the main factor affecting urine leakage following kidney transplantation.
