تغییرات Anti-MCV بعد از درمان در بیماران مبتلا به آرتریت روماتویید و ارتباط آن با اندکس فعالیت بیماری

چکیده

مقدمه: برخی مطالعات اندازه‌‌گیری تیتر Anti-MCV (Anti-mutated citrullinated vimentin antibodies) را به عنوان یک تست اختصاصی برای تشخیص و پیشرفت بیماری آرتریت روماتویید شناخته و سطح آن را با نمره‌ی فعالیت بیماری (Disease Activity Score Calculator for Rheumatoid Arthritis یا DAS28) مرتبط دانسته‌اند. هدف ما از انجام این مطالعه، بررسی ارتباط تیتر این آنتی‌‌بادی با فعالیت بیماری و تغییرات آن طی درمان بیماران مبتلا به بیماری آرتریت روماتویید (Rheumatoid arthritis یا RA) بود.روش‌ها: این مطالعه از نوع کارآزمایی بالینی بود. 80 بیمار مبتلا به RA به روش آسان وارد مطالعه شدند. سطح خونی ESR و Anti-MCV اندازه‌گیری و فرم DAS28 تکمیل گردید. پس از یک دوره‌ی درمانی 3 ماهه دوباره این مقادیر اندازه‌گیری شد. نتایج توسط آزمون آماری Student-t، Pearson و نرم‌افزار SPSS نسخه‌ی 15 آنالیز گردید.یافته‌ها: میانگین Anti-MCV قبل و بعد از درمان در افراد مورد مطالعه تفاوت معنی‌داری با یکدیگر داشت (001/0 > P). آزمون آماری Pearson ارتباط معنی‌داری را بین DAS28 و سطح Anti-MCV قبل و بعد از درمان نشان داد (05/0 > P).نتیجه‌گیری: تست Anti-MCV باید به عنوان تست سرولوژیکی مطلوب، که قادر به تشخیص RA در مراحل اولیه بیماری است، در نظر گرفته شود. Anti-MCV با فعالیت بیماری نیز در ارتباط است.

Background: In some studies Anti-mutated citrullinated vimentin antibodies (Anti-MCV) was known as a specific marker to diagnose and evaluate the process of rheumatoid arthritis (RA). In this study our aim was to evaluate the relationship between the disease activity score (DAS28) and serum titer of Anti-MCV in patients with rheumatoid arthritis.Methods: This study was a clinical trial in which 80 patients with RA were enrolled. All patients were treated for 3 months. ESR, Anti-MCV and DAS28 were measured before and after the treatment. Data was analyzed be Paired t-test and χ2.Findings: Anti-MCV titer was decreased significantly after treatment (P < 0.001). Pearson test showed a significant correlation between DAS28 and Anti-MCV titer (P < 0.05).Conclusion: Our findings suggested that Anti-MCV should be one of the serelogic tests for evaluating DAS28 in patients with rheumatoid arthritis.
