چکیده
مقدمه: استافیلوکوکوس اورئوس یکی از پاتوژنهای خطرناکی است که عامل بسیاری از بیماریهای عفونی میباشد. به همین دلیل تشخیص، کنترل، شناسایی دقیق و درمان این باکتری بسیار حایز اهمیت میباشد. هدف از این مطالعه، راهاندازی روشی سریع با تکنیک Multiplex PCR برای تشخیص به موقع جهت ارائهی راهکار لازم برای مقابله با باکتری استافیلوکوکوس اورئوس بود. روشها: در این مطالعه از 225 نفر از کارکنان شاغل در بیمارستانهای آموزشی و درمانی شهر زنجان به روش تصادفی نمونهگیری از قسمت قدامی بینی انجام شد. پس از انجام تستهای شناسایی و تأیید باکتری مورد نظر، جهت انجام آنتیبیوگرام از محیط کشت مولر هینتون آگار به روش دیسک دیفیوژن Kirby-Bauer استفاده شد. پس از استخراج DNA به روش جوشان و روش فنل کلروفورم، ژنهای femA، mecA و Sa442 (قطعهای از کروموزوم اختصاصی باکتری) به طور همزمان با دستگاه ترموسایکلر تکثیر شد و پس از انجام ژل الکتروفورز و رنگآمیزی با SyberGreen DNA باندها شناسایی شدند. یافتهها: از 225 نمونه، در 208 نمونه رشد کلنی مشاهده شد. از 208 کلنی، 203 نمونه گرم مثبت، 201 نمونه کاتالاز مثبت و 71 نمونه کوآگولاز مثبت گزارش شد. نتایج آنتیبیوگرام نشان داد که باکتری استافیلوکوکوس اورئوس به آنتیبیوتیکهای اگزاسیلین و پنیسیلین بیشترین مقاومت و به آمیکاسین و ونکومایسین بیشترین حساسیت را دارد. با بهینهسازی شرایط PCR (Polymerase chain reaction) ژنهای femA، mecA و Sa442 در یک میکروتیوب به صورت Multiplex تکثیر شدند. نتیجهگیری: روش Multiplex PCR به عنوان یک تست سریع و دقیق در مقایسه با روش دیسک دیفیوژن میتواند در شناسایی استافیلوکوکوس اورئوسهای مقاوم به متیسیلین کمک کننده باشد. واژگان کلیدی: استافیلوکوکوس اورئوس، mecA، femA، MRSA، Multiplex PCR

Introduction: Staphylococcus aureus is one of the pathogens that is a major agent of infectious diseases. So detection, control and treatment of this pathogen is very important. The objective of this study is setting up a fast method to detect methicillin resistant Staphylococcus aureus (MRSA) for better management of diseases related to this pathogen.Materials and methods: In this study 225 sample collected form staff of University hospitals of Zanjan province. After detection and confirmation of bacteria by conventional and biochemical methods, antimicrobial susceptibility was tested by disk diffusion (Kirby-Bauer) method. After DNA extraction by boiling and phenol – chloroform method, Sa442, mecA and femA genes were detected by multiplex PCR and PCR products were analyzed by electrophoresis (70V, 30 min) in gels composed of 1.5% (w/v) agarose stained with SyberGreen.Result: 208 out of 225 samples made colony in bacterial culture medium. 203/208 isolates were gram positive; 201/208 isolates were catalase positive and 71/208 isolates were coagulase positive. Most of the isolates were resistant to oxacillin and penicillin. By optimization of PCR conditions, Sa442, mecA and femA genes were detected by multiplex PCR simultaneously in single reaction tube.Discussion: Multiplex PCR can detect rapidly and selectively methicillin resistant Staphylococcus aureus in comparison with disk diffusion method.
