اثر انالاپریل در بهبود وضعیت سیتوکین‌های ضد التهابی در بیماران مبتلا به میگرن

چکیده

مقدمه: میگرن یکی از انواع شایع سردرد است. یکی از مکانیسم‌های احتمالی در فیزیوپاتولوژی این بیماری التهاب نوروژنیک می‌باشد که در مطالعات قبلی با تغییراتی که در میزان انواع فاکتورهای التهابی نظیر اینترلوکین‌ها، Adhesion moleculeها و واکنش‌گرهای فاز حاد انجام شده، وجود آن مطرح گردیده است. از طرفی، یکی از عملکردهای مهارکننده‌های آنزیم مبدل آنژیوتانسین (Angiotensin converting enzyme inhibitors یا ACEIs) فعالیت ضد التهابی آن‌ها می‌باشد. هدف از انجام این مطالعه، بررسی اثر انالاپریل در وضعیت سیتوکین‌های ضد التهابی در بیماران مبتلا به میگرن بود.روش‌ها: در این کارآزمایی بالینی تصادفی شده‌ی دوسوکور با شاهد دارونما، تجویز 10 میلی‌گرم انالاپریل در روز در مقایسه با دارونمای هم‌سان در 40 نفر بیمار مبتلا به میگرن برای دو ماه انجام گرفت. میزان اینترلوکین 4 (IL-4)، اینترلوکین 10 (IL-10) و اینترلوکین 13 (IL-13) در سرم تمام بیماران در ماه‌های 0 و 2 اندازه‌گیری شد.یافته‌ها: مقادیر IL-4 (001/0 = P)، IL-10 (001/0 > P) و IL-13 (001/0 > P) در گروه مورد پس از درمان با انالاپریل افزایش معنی‌داری یافت.نتیجه‌گیری: انالاپریل ممکن است در بهبود وضعیت التهابی بیماران مبتلا به میگرن مؤثر باشد؛ این اثر ضد التهابی می‌تواند در اثر افزایش سیتوکین‌های ضد التهابی در این بیماران ایجاد شده باشد. انالاپریل با کاهش خطر درگیری‌های مغزی- عروقی می‌تواند شانس ایجاد حملات بعدی میگرن و همچنین سایر بیماری‌های عروقی وابسته را بکاهد.

Background: Migraine is one of the most common forms of headache. One of its possible mechanisms is neuro-inflammation in which some changes happen in inflammatory factors such as interleukins, adhesion molecules or acute phase reactants. On the other hand, one of the mechanisms by which angiotensin converting enzyme inhibitors (ACEIs) affect is anti-inflammation. The goal of this study was to evaluate the effect of enalapril on anti-inflammatory cytokines of patients with migraine.Methods: In this randomized double-blind placebo-controlled clinical trial, 10 mg enalapril daily was compared with matched placebo in 40 patients with migraine for two months. Serum IL-4, IL-10 and IL-13 levels were measured in all patients at 0 and 2 month.Findings: The serum level of IL-4 (P = 0.001), IL-10 (P < 0.001) and IL-13 (P < 0.001) increased significantly in the case group after treatment with enalapril.Conclusion: Enalapril may be effective in improvement of inflammatory responses in patients with migraine. This improvement could be the result of anti-inflammatory cytokines and may reduce the cerebrovascular risks in these patients.
