سلول‌های T تنظیمی در ضایعات حاد و مزمن لیشمانیوز جلدی ناشی از انگل لیشمانیا ماژور

چکیده

مقدمه: لیشمانیوز جلدی در بسیاری از کشورهای جهان یک مشکل بهداشتی مهم محسوب می‌شود. مطالعاتی که در مدل‌های حیوانی و همچنین، در انسان انجام شده است، نشان می‌دهد که نوع پاسخ ایمنی در محل ضایعه، به خصوص تعادل بین سلول‌های T تنظیمی و سلول‌های T عملگر بر سیر بیماری موثر است و می‌تواند مشخص کند که بیماری خودبه‌خود بهبود می‌یابد یا این که به فرم‌های مزمن تبدیل می‌شود. هدف از این مطالعه، بررسی نقش سلول‌های T تنظیمی طبیعی (nTreg) در ضایعات حاد و مزمن لیشمانیوز جلدی ناشی از لیشمانیا ماژور بود.روش‌ها: از ضایعات جلدی 28 بیمار مبتلا به لیشمانیوز جلدی که از نظر آزمایشگاهی ابتلای آن‌ها به لیشمانیا ماژور ثابت شده بود، بیوپسی گرفته شد. این بیماران در دوگروه افراد مبتلا به ضایعات حاد و افراد مبتلا به ضایعات مزمن تقسیم شدند. میزان بیان ژن forkhead box P3 (foxp3) با روش Real-time PCR (Real-time reverse transcription polymerase chain reaction) و تعداد سلول‌های nTreg با روش رنگ آمیزی ایمونوفلورسنت در ضایعات بیماران دوگروه بررسی شد. از آزمون آماری  Mann-Whitney U برای تعیین معنی‌داری تفاوت‌ها بین دو گروه استفاده شد.یافته‌ها: میانگین بیان نسبی ژن foxp3 در ضایعات حاد و مزمن به ترتیب 23/0 ± 53/0 و 99/0 ± 26/1 بود که به صورت معنی‌داری در ضایعات مزمن بیشتر بود (007/0 = P). در روش رنگ‌آمیزی بافتی ایمونوفلورسنت نیز تعداد سلول‌های nTreg ضایعات مزمن به صورت معنی‌داری بیشتر از ضایعات حاد بود.نتیجه‌گیری: افزایش بیان ژن و همچنین بیان نشانگرهای پروتئینی سلول nTreg در ضایعات مزمن لیشمانیوز جلدی نشان می‌دهد که احتمال دارد این سلول‌ها در مزمن شدن ضایعات نقش داشته باشند و در نتیجه ممکن است بتوان از روش‌های ایمونوتراپی که هدف آن‌ها nTreg است، برای درمان این ضایعات مزمن استفاده کرد.

Background: Cutaneous leishmaniasis (CL) is a public health problem in several endemic countries. Recent studies on mouse model and a few clinical experiments showed that the type of immune response generated at the site of infection and especially balance between regulatory and effector T-cells determines the outcome of the disease toward self-limiting or long-lasting lesions. The aim of this study was to evaluate the role of natural regulatory T-cells (nTregs) in early and late cutaneous lesions of human Leishmania major (L. major) infection.Methods: Skin biopsies were collected from parasitologically proven lesions of 28 patients with cutaneous leishmaniasis, divided into two groups of early and late lesions. The causative agents were identified to be L. major. Quantitative real-time reverse transcription polymerase chain reaction (PCR) and immunofluorescent staining of biopsies were used to assess the Foxp3 mRNA expression and frequency of nTregs in two groups. Mann-Whitney U test was used to determine the significance of deferences between the two groups.Findings: Mean relative expressions of Foxp3 mRNA were 0.53 ± 0.23 and 1.26 ± 0.99 in early and late lesions, respectively, which was significantly upper in chronic lesions (P = 0.007). Parallel results were obtained in tissue staining method.Conclusion: Increased in gene expression and protein staining of nTreg markers in chronic biopsy samples indicates a role for these cells in chronic L. major induced leishmaniasis and supports the effectiveness of regulatory T cell-based immunotherapy for treatment of chronic cutaneous leishmaniasis.
