شیوع دیابت بارداری در ایران: مرور سیستماتیک

چکیده

مقدمه: دیابت بارداری یک مشکل بهداشتی در حال افزایش در سراسر دنیا و یکی از شایع‌ترین عوارض حاملگی می‌باشد که آثار متعددی بر مادر و جنین می‌گذارد. با توجه به این که اطلاعات زیادی در مورد شیوع دیابت بارداری در کشور در دسترس نبود و تفاوت‌های زیادی در نتایج مطالعات انجام شده مشاهده شد، مطالعه‌ی حاضر به صورت نظام‌مند و به منظور بررسی شیوع دیابت بارداری در ایران انجام گرفت.روش‌ها: مطالعه‌ی حاضر یک مطالعه‌ی مرور سیستماتیک در مورد شیوع دیابت بارداری در ایران می‌باشد. با جستجو در بانک‌های اطلاعاتی PubMed، Google scholar، Medlib، Magiran و SID (پایگاه جهاد دانشگاهی) و پس از تکمیل جستجو و ارزیابی مقالات با استفاده از چک لیست، تعداد 20 پژوهش که در سال‌های 91-1370 در ایران انجام شده بود، وارد مطالعه گردید.یافته‌ها: به طور کلی در مطالعه‌ی حاضر شیوع دیابت بارداری 88/5 درصد در کشور برآورد گردید. میزان شیوع دیابت بارداری در بین مطالعات مختلف از 6/18-3/1 درصد متغیر بود. کمترین میزان شیوع دیابت بارداری در اردبیل 3/1 درصد و بیشترین میزان دیابت بارداری در کرج 6/18 درصد مشاهده شد.نتیجه‌گیری: شیوع دیابت بارداری در کشور رو به افزایش است و بررسی دیابت بارداری در تمام استان‌های کشور با روش غربالگری و تشخیصی یکسان به منظور برآورد صحیح از شیوع دیابت بارداری در کشور و همچنین در اختیار قرار دادن اطلاعات دقیق‌تر جهت برنامه‌ریزی سیاست‌گذاران سلامت امری ضروری به نظر می‌رسد.

Background: Gestational diabetes mellitus (GMD) is a growing health problem around the world and is one of the most common complications in pregnancy that have negative impact on mother and newborn. According to the lack of enough available information about the prevalence of gestational diabetes mellitus in Iran and the differences in the results of studies, this systematic study aimed to clear the issue.Methods: A systematic search conducted in PubMed, Google scholar, Medlib, Magiran, and SID databases. Then, the found articles were evaluated using a checklist. 20 articles in the years 1995 to 2013 were enrolled in study.Findings: The overall prevalence of gestational diabetes mellitus was measured as 5.88% in Iran. The prevalence varied among different studies from 1.3% to 18.6%. The lowest prevalence was in Ardabil (1.3%), and the highest was in Karaj (18.6%).Conclusion: The findings show that the prevalence of gestational diabetes mellitus in Iran is increasing. Evaluating the prevalence in all provinces with identical diagnostic screening method is necessary to estimate the true prevalence of gestational diabetes mellitus as well as to provide more accurate information to health policy makers .
