گزارش یک مورد ماستیت بروسلایی در یک خانم یائسه

چکیده

مقدمه: : بیماری بروسلوز به صورت آندمیک در سراسر ایران دیده می‌شود. این بیماری توانایی ایجاد علایم به صورت سیستمیک را دارد، اما بعضی از اندام‌‌ها به ندرت درگیر می‌شوند. ابتلای بافت پستان در حیوانات اغلب گزارش می‌شود، اما در انسان عارضه‌ی نادری است.معرفی بیمار: بیمار خانم 67 ساله ساکن روستایی در بابل، دامدار که با علایم تب و لرز، تورم، قرمزی و درد یک طرفه‌ی پستان راست مراجعه نمود. از یک هفته قبل از مراجعه، بیمار دچار تب، ضعف و بی‌حالی، درد مفاصل و بی‌اشتهایی بود. در معاینات بالینی تورم، قرمزی و ایندوراسیون در پستان مشاهده شد. بیماری‌های بدخیم پستان مطرح گردید. گزارش اولترا سونوگرافی ماستیت بود. به دنبال عدم پاسخ به آنتی‌بیوتیک‌های بتالاکتام، آزمایش‌های سرولوژی انجام شد و نتایج آزمایش‌های رایت (Wright) 640/1، کومبس رایت (Coombs- Wright) 640/1 و 2- مرکاپتو اتانول (Mercaptoethanol-2) 320/1 بود. بیمار تحت درمان با دوکسی سیکلین و استرپتومایسین قرار گرفت. بعد از دو ماه دریافت دارو، بیمار پیگیری شد. علایم بالینی بهبود یافت و یافته‌های سونوگرافی مشاهده نشد.نتیجه‌گیری: با امکان مطرح نمودن بیماری‌های بدخیم پستان در شرایط موجود و در نواحی آندمیک، به علت هماتوژن بودن بروسلا و درگیری در هر اندام از بدن، باید این بیماری جزء تشخیص‌های افتراقی قرار گیرد.

Background: Brucellosis is an endemic disease in Iran. It produces systemic symptoms in humans but organs such as breasts are rarely involved. Breast involvement of brucella can be frequently detected in animals; however, it is extremely rare in humans.Case Report: A 67-year-old woman, living in rural area in Babol, Iran, with fever, trembling and inflammation on one side of right breast referred to a clinic. She had fever, weakness, arthritis and loss of appetite from one week prior to the visit. Ultrasound was requested and showed mastitis. Antibiotics in beta-lactam groups had no effect on the patient. Serology tests of Wright, Combs-Wright and 2-mercaptoethanol were positive with the titer of 1/640, 1/640 and 1/320, respectively. Treatment with a combination of doxycycline and streptomycin lead to clinical care. She was under observation for 2 months until her ultrasound results changed to negative.Conclusion: In endemic area, brucellosis should be pointed out; as it is clinically difficult to distinguish this type of infection from an inflammatory carcinoma of the breast or any other organs.
