اپیدمیولوژی وتابلوی بالینی لیشمانیوز جلدی در شهر بادرود سال 1392

چکیده

مقدمه: لیشمانیوز جلدی، جزء بیماری‌های انگلی اندمیک شایع در ایران به شمار می‌رود و یکی از معضلات مهم بهداشتی در استان اصفهان و به ویژه شهر بادرود می‌باشد که شیوع آن در دهه‌ی گذشته دو برابر شده است. این مطالعه، به منظور تعیین اپیدمیولوژی و تابلوی بالینی لیشمانیوز جلدی در شهرستان بادرود در سال 1392 انجام شد.روش‌ها: این مطالعه‌ی توصیفی- مقطعی بر روی کلیه‌ی بیماران مبتلا به سالک شناسایی شده از طریق بیماریابی فعال در شهر بادرود در طی یک سال انجام شد. بیماران توسط پزشک ویزیت شدند و پرسش‌نامه‌ی حاوی اطلاعات دموگرافیک و تابلوی زخم، از طریق مصاحبه و معاینه‌ی بیماران تکمیل گردید. تشخیص، با آزمایش زخم از نظر جسم لیشمن انجام گرفت. اطلاعات با نرم‌افزار SPSS مورد تجزیه و تحلیل قرار گرفت.یافته‌ها: 63 بیمار مبتلا به لیشمانیوز جلدی، طی یک سال در شهر بادرود شناسایی شدند.6/55 درصد بیماران مرد بودند. گروه سنی 30-15 سال بیشترین فراوانی (8/23 درصد) را در بین مبتلایان داشت. 7/66 درصد بیماران سابقه‌ی اقامت در منطقه‌ی آقاعلی‌عباس را داشتند. تمامی زخم‌ها از نوع روستایی بود. بیشتر ضایعات لیشمانیوز جلدی، زخمی با ابعاد کمتر از 3 سانتی‌متر، بدون درد و همراه با خارش‌ و ترشح بودند. اندام‌ها شایع‌ترین محل ابتلا بود. اغلب موارد مبتلا، پس از درمان بهبودی کامل یا نسبی داشتند.نتیجه‌گیری: در این مطالعه، مشخص شد که در شهر بادرود، لیشمانیوز جلدی از نوع روستایی بود و در مردان و سنین بالای 15 سال شیوع بیشتری داشت. غالب موارد ابتلا پس از درمان بهبودی کامل یا نسبی داشتند.

چکیده [English]

Background: Cutaneous Leishmaniasis (CL) is a common endemic parasitic disease in Iran. This disease is always a serious health problem in Isfahan province, especially in Badrood city, and its prevalence has been doubled over the last decade. This study was designed to determine the epidemiology and clinical features of cutaneous leishmaniasis in Badrood city in 2013.Methods: This descriptive cross-sectional study was done on all detected patients with cutaneous leishmaniasis in Badrood city via active detection during one year. Patients were visited by physician and a questionnaire including the demographic data and characteristics of the wounds were filled through interview and examination of patients. Diagnosis was confirmed via revealing of the leishman body in smear of the wounds. The results were analyzed using SPSS software.Findings: 55.6% of the patients were men. Age group of 15-30 years had the highest rate (23.8%) among the patients. 66.7% of patients had history of staying in Agha Ali Abbas region. All the wounds were of the rural form. The lesions of cutaneous leishmaniasis were painless, pruritus, less than 3 cm, and with secretion. Extremities were the most common sites for the infection. There was complete or partial healing after the treatment in most of the patients.Conclusion: Based on the findings of this study, cutaneous leishmaniasis in Badrood city is rural form and most of the patient are men and older than 15 years of age. Complete or partial healing can be seen after the treatment in most of the patients.
