چکیده
مقدمه: الکتروپوریشن روشی مؤثر برای هدایت داروهای درمانی (از جمله داروهای شیمی درمانی) به داخل سلولهای زنده است که به کاهش حجم و درمان مورد نظر در بافت میانجامد. کارایی این روش، وابسته به انتخاب بهینهی پارامترهای شکل موج تخریک است. این مطالعه، در جهت بهینهسازی پارامترهای درمان تومورهای سارکوم بافت نرم با روش الکتروپوریشن بازگشت ناپذیر انجام گرفت. روشها: هندسهی بافت و تومور در نرمافزار Mimics از تصاویر MRI (Magnetic resonance imaging) تومور و بافتهای سالم اطراف آن استخراج شد. سپس، با استفاده از نرمافزار CAD-fix، تبدیل هندسهی حاصل از نرمافزار Mimics به هندسهی قابل استفاده در نرمافزار المان محدود (COMSOL Multiphysics) انجام شد. به منظور حل معادلات مربوط (معادلهی Laplace و معادلهی بیوگرمایی) به روش المان محدود، از نرمافزار FEMLAB نسخهی 4/4 استفاده شد. همچنین، از FEMLAB جهت برآورد توابع هدف در قسمت بهینهسازی نیز استفاده شد و به منظور بهینهسازی هندسهی الکترودهای مورد استفاده در الکتروپوریشن بازگشت ناپذیر، از الگوریتم ژنتیک بهره جستیم. کد الگوریتم ژنتیک با استفاده از نرمافزار MATLAB نسخهی 7.10.0 نگارش شد.یافتهها: شدت میدان الکتریکی اعمالی به تومور با افزایش ولتاژ اعمالی به الکترودها افزایش یافت و بیشترین شدت توزیع میدان الکتریکی، با اختلاف پتانسیل 4000 ولتی مشاهده شد. با تغییر عمق ورود الکترود در بافت، فاصلهی بین الکترودها و همچنین ضخامت الکترودها، میانگین شدت میدان الکتریکی توزیعی در بافت دچار تغییر شد. با وجود میدان الکتریکی بالا در ناحیهی تومورال، میانگین توزیع حرارتی در تومور فقط در حد چند درجهی سانتیگراد نسبت به حالت معمول (37 درجهی سانتیگراد) افزایش یافت؛ گرچه در نقاط نزدیک به انتهای الکترودها، امکان افزایش حرارت تا 600 درجهی سانتیگراد نیز بود.نتیجهگیری: با استفاده از مدلسازی عددی و لحاظ هندسهی واقعی بافت و تومور میتوان پارامترهای بهینهی مؤثر در درمانهای الکتروپوریشن بازگشت ناپذیر را برای کنترل رشد تومور و کنترل آسیب به بافتهای سالم محاسبه کرد.

Background: Electroporation is an effective strategy for drug delivery to the target volume, for example in chemotherapy. The efficiency of this method depends on the selection of optimal parameters affecting it. This studay aimed to evaluate the optimum parameters for treatment of soft tissue sarcoma tumors on irreversible electroporation treatments.Methods: The geometry of the tumor was defined using the Mimics software regarding to the gatherd magnetic resonance images (MRI). Using CAD-fix software, the Mimics product geometry was converted to the geometry that could be used in the finite element. The FEMLAB4.4 software utilized to solve equations (Laplace and the biothermal equations). In order to optimize the geometry of the electrodes used in irreversible electroporation, a Genetic Algorithm program was used. The Genetic Algorithm code was written using the MATLAB7.10.0 software.Findings: The induced electric field intensity to the tumor increased with increasing voltage applied to the electrodes and the maximum electric field intensity was observed in the potential of 4000 volts. The average electric field intensity distribution in the tissue was altered by changing the electrode penetration depth, the distance between the electrodes and the thickness of the electrodes. Despite the high electric field utilized to the tumor area, there was only a slight increase in the average temperature of the tumor (37°C). Although the tissue temperature near the end of the electrode could be increased up to 600°C.Conclusion: The optimum effective parameters of irreversible electroporation treatments for tumor ablation could be calculated using the numerical modeling methods.
