آیا تفاوتی از نظر سطح آنتی‌بادی ضد عفونت هلیکوباکترپیلوری در بین دو میگرن با اورا و بدون اورا وجود دارد؟

چکیده

مقدمه: مطالعات اخیر وجود همبستگی مثبت بین عفونت هلیکوباکترپیلوری و سردردهای میگرنی را نشان داده است. هدف این مطالعه، ارزیابی نقش عفونت هلیکوباکترپیلوری در سردردهای میگرنی با اورا (MA) و بدون اورا (MO) بود.روش‌ها: در این مطالعه‌ی مورد- شاهدی، 84 بیمار (از هر دو گروه با و بدون اورا) و 49 شخص سالم مورد بررسی قرار گرفتند. از آزمون ELISA (Enzyme-linked immunosorbent assay) برای اندازه‌گیری میزان ایمنوگلوبولین G (IgG) و ایمنوگولوبولین M (IgM) در هر دو گروه، استفاده شد. شدت سردردها بر اساس پرسشنامه‌ی اثر سردرد (HIT6 یا Headache Impact Test-6) ارزیابی شد.یافته‌ها: میانگین سن در بیماران و گروه شاهد به ترتیب 1/11 ± 8/35 و 9/18 ± 4/33 سال بود. میانگین میزان آنتی‌بادی IgM در بیماران (U/ml 1/23 ± 3/26) به نحو معنی‌داری بیشتر از گروه شاهد (U/ml 2/11 ± 5/17) بود (004/0 = P). در گروه بیماران، تفاوت چشم‌گیری در میانگین نمره‌ی HIT6 بین گروه‌های با اورا (7/4 ± 5/65) و بدون اورا (3/5 ± 9/54) مشاهده شد (001/0 > P). همبستگی قابل ملاحظه‌ای بین آنتی‌بادی IgG و نمره‌ی HIT6 در بیماران با اورا (011/0 = P؛ 407/0 = r) و بدون اورا (002/0 = P؛ 499/0 = r) یافت شد. تفاوت معنی‌داری نیز بین سطوح طبیعی و بالای آنتی‌بادی IgG و شدت سردرد در تمامی بیماران مشاهده شد (002/0 = P). خطر وقوع میگرن در بیماران، ارتباط چشم‌گیری با سطوح آنتی‌بادی IgG و IgM نداشت.نتیجه‌گیری: نتایج، این نوید را می‌دهد که درمان قطعی و ریشه کن کردن باکتری هلیکوباکترپیلوری بتواند، یک راه درمانی برای کاهش شدت و دوره‌ی سردردهای میگرنی باشد.

Is There Any Difference in the Levels of Antibodies against Helicobacter Pylori Infection between the Patients with Migraine Headache with or without Aura?

Background: Recent studies have focused on a positive correlation between Helicobacter pylori (H. pylori) infection and occurrence of migraine headache. This study aimed to determine the role of H. pylori infection in the two common types of migraine headache including with (MA) and without aura (MO).Methods: In a case-control study, 84 patients (with MA or MO migraine headache) and 49 healthy individuals were enrolled. The enzyme-linked immunosorbent assay (ELISA) was used to measure the levels of immunoglobulin G (IgG) and M (IgM) against H. Pylori in patients and controls.Findings: The mean age was 35.8 ± 11.1 and 33.4 ± 18.9 years in patient and control groups, respectively. The mean level of IgM antibody was significantly more in patients (26.3 ± 23.1 U/ml) compared to control group (17.5 ± 11.2 U/ml) (P = 0.004). It was not any significant difference between the levels of IgM and IgG antibodies in patients with MA or MO migraine headache. In addition, the mean HIT6 score was significantly more in patients with MA (65.5 ± 4.7) compared to those with MO (54.9 ± 5.3) migraine headache (P < 0.001). The only significant correlation was found between the IgG antibody level and the HIT6 score in patients with MA (r = 0.407; P = 0.011) and MO (r = 0.499; P = 0.002) migraine headache. There was significant relationship between the normal and high levels of IgG antibody and the severity of headache (P = 0.002) in all the patients. The risk of migraine occurrence in patients was not significantly associated with the levels of IgG and IgM antibodies.Conclusion: Further investigation should be designed to gain better conclusion about the role of H. Pylori treatment and eradication in the course of migraine headaches.
