مقایسه‌ی اثرات همودینامیک انفوزیون تری‌نیتروگلیسرین و لابتالول در کنترل فشار خون و ضربان قلب، قبل و حین اینداکشن تا زمان ارست قلبی در بیماران تحت جراحی بای‌پس عروق کرونر

چکیده
مقدمه: مطالعه‌ی حاضر با هدف مقایسه‌ی اثرات همودینامیک انفوزیون تری‌نیتروگلیسرین و لابتالول در کنترل فشار خون و ضربان قلب، قبل و حین اینداکشن تا زمان ارست قلبی در بیماران تحت جراحی بای‌پس عروق کرونر (Coronary artery bypass grafting یا CABG) انجام شد.روش‌ها: در این مطالعه‌ی کارآزمایی بالینی، 70 بیمار کاندید عمل جراحی CABG، در دو گروه 35 نفره‌ی دریافت کننده‌ی انفوزیون نیتروگلیسرین و لابتالول قرار گرفتند. تغییرات همودینامیک، سطح گازهای خونی و بروز آریتمی‌های قلبی حین عمل و همچنین، میزان نیاز به داورهای آنتی‌آریتمیک، اینوتروپ، پک‌سل و DC شوک در دو گروه تعیین و مقایسه گردید.یافته‌ها: بروز آریتمی‌های قلبی در گروه نیتروگلیسرین، 9 مورد (7/25 درصد) و در گروه لابتالول 5 مورد (3/14 درصد) گزارش شد (370/0 = P). میزان نیاز به داروهای آنتی‌آریتمیک در دو گروه نیتروگلیسرین و لابتالول به ترتیب 9/22 و 9/2 درصد بود که اختلاف معنی‌داری بین دو گروه مشاهده شد (028/0 = P).نتیجه‌گیری: در عمل جراحی CABG، استفاده از لابتالول نسبت به نیتروگلیسرین ارجحیت دارد و در صورت عدم وجود منع مصرف، استفاده از آن در این عمل جراحی پیشنهاد می‌گردد.

Background: This study was aimed to compare the hemodynamic effects of nitroglycerin and labetalol infusion for hypertension and heart rate control in the before and during induction to cardiopulmonary arrest in patients under coronary artery bypass graft.Methods: In a clinical trial study, 70 candidates for coronary bypass were selected and randomly divided in the two groups. In the first group nitroglycerin and in the second group labetalol was injected and hemodynamic changes, blood gas levels, incidence of arythmia during and after surgery was measured. Also amount of anti-arythmic drugs, inotrop, pack cell and DC shock need were studied and compared between the two groups.Findings: Incidence of cardiac arrhythmia in the nitroglycerin and labetalol was 9 and 5 (25.7% VS 3.14%) (P = 0.370), also need to anti-arrhythmic drugs in the two groups was 22.9% and 2.9%, respectively (P = 0.028).Conclusion: Using labetalol in patients under coronary artery bypass graft is better than nitroglycerin for hemodynamic and blood gases. Also, incidence of cardiac arrhythmia during cardiopulmonary arrest is lower and labetalol is preferred to nitroglycerin and recommended according to surgeons and anesthesiologists.
