چکیده

مقدمه: در جراحی تومورهای کولورکتال به روش لاپاراسکوپی، کلیپ‌هایی از جنس تیتانیوم برای عمل جراحی استفاده می‌شوند. زمانی که این کلیپ‌ها در مسیر پرتوهای پر انرژی جهت پرتودرمانی قرار می‌گیرند، می‌توانند موجب اختلال در دز جذبی شوند. هدف از انجام این مطالعه، بررسی تأثیر این کلیپ‌های تیتانیومی در توزیع دزهای پرتودرمانی با فوتون‌های پرانرژی 6 و 18 مگاولت با استفاده از نرم‌افزار طراحی درمان (Treatment planning software یا TPS) بود.روش‌ها: کلیپ‌های لاپاراسکوپی در فانتوم معادل بافت، مشابه روده‌ی لاپاراسکوپی شده قرار داده شدند. سپس، با استفاده از نرم‌افزار طراحی درمانی در انرژی‌های 6 و 18 مگاولت، توزیع دزها به دست آمد و نمودارهای درصد دز عمقی (Percentage depth dose یا PDD) و پروفایل رسم شدند. همچنین، با استفاده از توزیع دز محاسبه شده، دقت محاسبه‌ی پراکندگی به عقب ارزیابی شد.یافته‌ها: با بررسی نمودارها، تغییرات دز پرتویی در مسیر کلیپ‌ها ناشی از پرتوی پراکنده شده به عقب و اثر تضعیف مشاهده شد. بیشترین تغییرات ناشی از اثر تضعیف بعد از کلیپ تیتانیومی به ترتیب 7/1 و 1/1 درصد و تغییرات ناشی از پراکنده شده به عقب به ترتیب 4/0 و 7/0 درصد برای پرتوهایی 6 و 18 مگاولت به دست آمد.نتیجه‌گیری: در صورت حضور کلیپ‌های لاپاراسکوپی در مسیر پرتوهای پر انرژی، نمودار درصد دز عمقی و پروفایل دز تغییر می‌کنند و با وجود این که این تغییرات دز در محدوده‌ی یک کلیپ ناچیز است، برای حالتی که تعداد زیادی از کلیپ‌ها در مسیر پرتو قرار می‌گیرند، اثر تضعیف، قابل چشم‌پوشی نیست.

Background: In laparoscopy of colorectal tumors, titanium clips are used to close the surgery site. For post-operation treatment with radiation, the clips might change the dose distribution. In this study, the effect of titanium clips in isodose distribution of 6- and 18-MV photons was evaluated.Methods: Laparoscopy clips were placed on a phantom with material close to soft tissue. Computed tomography (CT) scan images of this phantom were imported in planning software and the percentage depth dose (PDD) and profiles of the dose were obtained for 6- and 18-MV photons.Findings: Calculated PDD showed weakening and backscattering of the dose of radiation in the path after the clips. Dose changes for the regions after the clips had a maximum of 1.7% and 1.1% for 6- and 8-MV photons, respectively. The amount of backscattering was up to 0.4% and 0.7% for 6- and 18-MV photons, respectively.Conclusion: Presence of the titanium clips in the path of radiation changes the depth dose and profiles. The changes for of a single clip are minor. However, for few clips beside each other, the effect of radiation attenuation is not negligible.
