تأثیر مشاوره‌ی جنسی بر اساس مدل PLISSIT بر عملکرد جنسی زنان باردار: یک کارآزمایی بالینی تصادفی کنترل شده

چکیده

مقدمه: مشکلات جنسی در دوران بارداری، منشأ بسیاری از اختلافات بین فردی و زناشویی می‌باشد. الگوی تغییرات فعالیت جنسی در دوران بارداری، می‌تواند عملکرد جنسی و در نتیجه رضایت جنسی زن باردار را تحت تأثیر قرار دهد. مشاوره، می‌تواند مشکلاتی را که زن باردار در طی این دوران با آن روبه‌رو می‌شود، تا حد قابل قبولی کاهش دهد. مطالعه‌ی حاضر با هدف ارزیابی تأثیر مشاوره بر اساس مدل Permission, limited information, specific suggestions, intensive therapy (PLISSIT) بر عملکرد جنسی زنان باردار انجام شد.روش‌ها: پژوهش حاضر، یک مطالعه‌ی کارآزمایی بالینی بود که بر روی 80 زن باردار 26-24 هفته که حداقل دارای یک مشکل جنسی تأیید شده از طرف روانظپزشک بودند، انجام شد. افراد به صورت تصادفی به روش بلوک جای‌گشتی در دو گروه مورد و شاهد تقسیم گردیدند. گروه مورد، توسط مامای آموزش دیده، بر اساس مدل PLISSIT مشاوره دریافت کردند و گروه شاهد، تنها مراقبت‌های معمول بارداری توسط مامای درمانگاه را دریافت کردند. جهت گردآوری داده‌ها، از پرسش‌نامه‌ی اطلاعات فردی و مامایی و پرسش‌نامه‌ی شاخص عملکرد جنسی زنان (Female sexual function index یا FSFI) جهت بررسی عملکرد جنسی زنان باردار در مراحل قبل، دو و چهار هفته بعد از مداخله استفاده شد. 050/0 > P به عنوان سطح معنی‌داری در نظر گرفته شد.یافته‌ها: پس از تعدیل اثر قبل از مداخله، بین میانگین نمره‌ی عملکرد جنسی و تمام حیطه‌های آن در دو گروه مورد و شاهد، در مرحله‌ی چهار هفته بعد از مداخله، اختلاف معنی‌داری وجود داشت (050/0 > P).نتیجه‌گیری: با توجه به تأثیر مشاوره‌ی جنسی بر اساس مدل PLISSIT بر بهبود عملکرد جنسی زنان در دوران بارداری، مشاوره‌ی جنسی در دوران بارداری توصیه می‌شود. در ضمن، پیشنهاد می‌گردد که در مطالعات بعدی، تأثیر مشاوره‌ی جنسی در دوران بارداری بر رضایت جنسی نیز مورد سنجش قرار گیرد.

چکیده [English]

Background: Sexual problems during pregnancy are the source of many interpersonal and marital conflicts. The pattern of changes in sexual activity during pregnancy can have an impact on sexual function and thus on sexual satisfaction in pregnant women. Counseling can reduce pregnant women's sexual problems during this time greatly. This study was conducted to determine the effect of counseling based on the PLISSIT model (Permission, Limited Information, Specific Suggestions, Intensive Therapy) on sexual function in pregnant women.Methods: This randomized clinical trial study was conducted on 80 pregnant women with gestational age of 24-26 weeks, having at least one sexual problem confirmed by psychiatrists. They were divided randomly in basis of block permutation into two groups of intervention and control. The intervention group received counseling based on PLISSIT by a trained midwife and control group received regular prenatal care by a midwifery clinic. Demographic and midwifery, and Female Sexual Function Index (FSFI) questionnaire were used and completed for data collection before intervention, and 2 and 4 weeks after that. P-value of less than 0.05 was considered significant.Findings: After adjusting for pre-intervention score, there were significant differences between the mean score of sexual function, and all its domains, among experimental and control groups at the fourth week after the intervention (P < 0.05).Conclusion: As sexual counseling based on PLISSIT model improved sexual function in women during pregnancy in this research, using this model in primigravida is advised. Evaluation of this method in sexual satisfaction is recommended for future studies.
