بررسی فراوانی و اهمیت یافته‌های خارج قلبی توراسیک در بیماران مراجعه کننده به بیمارستان الزهرای (س) اصفهان جهت انجام Computed Tomography Angiography (CT Angiography) عروق‌ کرونری طی سال‌های 95-1394

چکیده
مقدمه: هدف از انجام مطالعه‌ی حاضر، بررسی شیوع و‌ شدت یافته‌های خارج قلبی تصادفی در CT angiography عروق کرونر بود.روش‌ها: در این پژوهش، بیماران مشکوک به بیماری عروق ‌کرونری به دنبال تزریق ماده‌ی حاجب، تحت Multidetector computed tomography (MDCT) قرار گرفتند. تصاویر به دست آمده از نظر یافته‌های خارج قلبی در چهار دسته شامل «یافته‌های نیازمند درمان، یافته‌های نیازمند بررسی، یافته‌های نیازمند پیگیری و یافته‌های بدون نیاز به اقدام» طبقه‌بندی‌ شد.یافته‌ها: یافته‌های خارج قلبی در 30/24 درصد از بیماران وجود داشت. بدین ترتیب، یافته‌های نیازمند درمان 94/1 درصد، یافته‌های نیازمند بررسی 02/8 درصد، یافته‌های نیازمند پیگیری 94/10 درصد و یافته‌های بدون نیاز به اقدام در 40/3 درصد بیماران مشاهده‌ گردید.نتیجه‌گیری: شیوع یافته‌های خارج قلبی در تصاویر CT angiography قابل توجه بود. بنابراین، توصیه می‌شود جهت یافتن پاتولوژی‌ها، تمام فیلدهای قابل مشاهده با دقت بررسی گردد.

Background: The aim of our study was to evaluate the prevalence of accidental extracardiac findings in multidetector computed tomography (MDCT) angiography of coronary arteries.Methods: Patients with suspected coronary artery disease were evaluated with multidetector computed tomography during injection of contrast material. Images were reviewed for extracardiac findings which were classified as therapy needed, further workup needed, follow-up needed, and no further action needed.Findings: Extracardiac findings were detected in 24.30% of patients. The frequency of the findings classified as needed therapy, needed workup, needed follow-up, and no further action needed was 1.94%, 8.02%, 10.94%, and 3.40%, respectively.Conclusion: The prevalence of noncardiac findings on cardiac multidetector computed tomography was noticeable. To avoid missing clinically important findings, all the fields of cardiac multidetector computed tomography should be evaluated carefully.
