بررسی اثربخشی درمان مبتنی بر پذیرش و تعهد بر نشانگرهای روان‌شناختی (استرس، کیفیت زندگی و راهبردهای مقابله‌ای) در بیماران مبتلا به دیابت نوع 2

چکیده

مقدمه: دیابت نوع 2، یکی از بیماری‌های مزمن است که اثرات جسمی و روانی عدیده‌ای برای فرد به دنبال دارد. پژوهش حاضر، با هدف بررسی اثربخشی درمان مبتنی بر پذیرش و تعهد (Acceptance and commitment therapy یا ACT) بر نشانگرهای روان‌شناختی بیماران مبتلا به دیابت نوع 2 انجام گرفت.روش‌ها: پژوهش حاضر، از منظر نحوه‌ی اجرا، نیمه‌آزمایشی با طرح پیش‌آزمون- پس‌آزمون با گروه شاهد و آزمون پی‌گیری بود. جامعه‌ی آماری، شامل تمامی بیماران مبتلا به دیابت مراجعه کننده به مرکز دیابت شهر کرمانشاه در سال 1395 بود که واجد پرونده‌ی درمانی بودند. 40 نفر با توجه به معیارهای شمول، به روش در دسترس انتخاب و به ‌صورت جایگزینی تصادفی در دو گروه 20 نفره‌ی مورد و شاهد قرار گرفتند. افراد گروه مورد، درمان مبتنی بر پذیرش و تعهد را دریافت نمودند، اما افراد گروه شاهد درمان خاصی دریافت نکردند. افراد شرکت کننده، پرسش‌نامه‌های راهبردهای مقابله‌ای Lazarus-Folkman، رویدادها و تغییرات زندگی خانوادگی (Family inventory of life events یا FILE) و فرم کوتاه پرسش‌نامه‌ی کیفیت زندگی بیماران مبتلا به دیابت (Diabetes quality of life-brief clinical inventory یا DQOL-BCI) را تکمیل نمودند. تحلیل داده‌ها، با استفاده از نرم‌افزار SPSS انجام گرفت.یافته‌ها: بین میانگین نمرات پس‌آزمون راهبردهای مقابله‌ای مسأله‌مدار و هیجان‌مدار و نیز استرس، بعد از حذف اثر پیش‌آزمون، در گروه‌های مورد و شاهد، تفاوت معنی‌داری وجود داشت (010/0 ˂ P)، اما نتایج حاکی از آن بود که در متغیر کیفیت زندگی، تفاوت معنی‌داری بین دو گروه مورد و شاهد وجود نداشت (050/0 < P).نتیجه‌گیری: بر اساس نتایج این مطالعه، می‌توان گفت که درمان مبتنی بر پذیرش و تعهد می‌تواند در کاهش مشکلات بیماران مبتلا به بیماری‌های مزمن نظیر دیابت، اثرگذار باشد.

چکیده [English]

Background: Type II diabetes is considered as one of the chronic illnesses and has several physical and mental effects on patient. This research aimed to study the effect of acceptance and commitment therapy on psychological markers in patients with type II diabetes.Methods: This was a quasi-experimental research with pretest/posttest design and follow-up evaluations. Statistical population of the research included all the patients with type II diabetes with registered medical records referred to Kermanshah Diabetes Center, Kermanshah City, Iran, in 2016. 40 patients were selected using convenience sampling method and were randomly placed in two equal groups of control and intervention. They answered Lazarous-Folkman coping strategies, family inventory of life events (FILE) and short form of diabetes quality of life-brief clinical inventory (DQOL-BCL) questionnaires. Data analysis was done using SPSS software.Findings: There were significant differences between the posttest mean scores of problem- and emotion-oriented coping strategies, and stress in intervention and control groups (P < 0.010). However, there was no significant differences in quality of life in intervention and control group (P > 0.050).Conclusion: According to the results, it can be said that acceptance and commitment therapy may reduce the problems in patients with chronic diseases such as type II diabetes.
